Analisis Persepsi Apoteker terhadap Manfaat ChatGPT dalam Praktik Pelayanan Farmasi Klinik pada Geriatri di Indonesia

Latar Belakang: Artificial Intelligence (AI) telah berkembang sangat pesat di Indonesia dengan 82% pengguna. Generative Pre-Trained Transformer (ChatGPT) ke enam terbanyak di Dunia. ChatGPT telah mulai digunakan dalam pelayanan farmasi klinis. Namun, data tentang persepsi dan praktik penggunaanya dalam pelayanan farmasi klinis dalam pelayanan geriatri di Indonesia masih terbatas.
Tujuan: Penelitian ini bertujuan untuk mengidentifikasi persepsi apoteker terhadap pelayanan farmasi klinis dengan ChatGPT dan ChatGPT sebagai teknologi baru, serta menganalisis hubungan antara persepsi apoteker dengan praktik penggunaannya.
Metode: Penelitian ini menggunakan desain cross-sectional dengan pendekatan kuantitatif yang melibatkan 104 apoteker di seluruh Indonesia. Instrumen penelitian ini diadaptasi dan dikembangkan dari penelitian terdahulu yang telah melalui uji validitas serta reliabilitas untuk memastikan kelayakannya. Data responden dianalisis secara univariat dan bivariat untuk mengetahui persepsi, praktik, hubungan persepsi apoteker dengan praktik penggunaanya serta hubungan karakteristik demografi dengan persepsi.
Hasil: Persepsi apoteker terhadap penerapan ChatGPT di pelayanan farmasi klinis pada pasien geriatri, baik pada populasi seluruh responden maupun kelompok yang telah menggunakan ChatGPT berada pada kategori positif. Apoteker mendukung pemanfaatan ChatGPT untuk membantu tugas yang tidak berkaitan langsung dengan pengambilan keputusan klinis yang kompleks. Sejalan dengan persepsi apoteker, praktik penggunaan ChatGPT dalam pelayanan farmasi klinis tergolong tinggi sebesar 57,9%. Pola penggunaan ChatGPT, apoteker cenderung lebih sering (sering; 43,9%) menggunakan ChatGPT untuk tugas edukatif dan mencari informasi obat atau penyakit. Sebaliknya, teknologi ini belum digunakan secara rutin (kadang-kadang) untuk identifikasi interaksi obat (33,3%), dan efek samping (33,3%) serta jarang digunakan (jarang; 42,1%) untuk penyesuaian dosis karena adanya kekhawatiran terhadap akurasi dan keamanan privasi. Sebagai langkah antisipasi, apoteker selalu (49,1%) melakukan verifikasi ulang informasi dari ChatGPT dengan sumber klinis terpercaya. Tidak terdapat hubungan persepsi dengan praktik penggunaan ChatGPT (p=0,954). Karakteristik sosiodemografi tingkat pendidikan berhubungan dengan persepsi apoteker pada aspek pelayanan farmasi klinis (p=0,025) dan pengalaman penggunaan berhubungan signifikan pada aspek ChatGPT sebagai teknologi baru (p=0,005).
Kesimpulan: Penelitian ini menunjukkan bahwa apoteker menerima baik dan aktif memanfaatkan ChatGPT meskipun membatasi penggunaan hanya pada tugas edukatif dan informatif akibat adanya kekhawatiran privasi data dan akurasi sehingga perlu penyusunan regulasi resmi dan pelatihan mitigasi risiko klinis.
Background: Artificial Intelligence (AI) has advanced rapidly in Indonesia, which ranks sixth globally with an 82?option rate of Generative Pre-trained Transformer (ChatGPT). While ChatGPT has begun to be integrated into clinical pharmacy services, data regarding pharmacists' perceptions and practices within geriatric care in Indonesia remain scarce.
Objective: This study aimed to identify pharmacists' perceptions toward clinical pharmacy services using ChatGPT and ChatGPT as an emerging technology, as well as to analyze the relationship between pharmacists' perceptions and their actual practices.
Method: This study employed a quantitative cross-sectional design involving 104 pharmacists across Indonesia. The research instrument was adapted and developed from previous studies, having undergone validity and reliability testing to ensure its feasibility. Respondent data were analyzed using univariate and bivariate analyses to evaluate perceptions, practices, the correlation between pharmacists' perceptions and utilization practices, and the relationship between sociodemographic characteristics and perceptions.
Results: Pharmacists' perceptions toward the implementation of ChatGPT in clinical pharmacy services for geriatric patients both among the overall respondents and the user group were predominantly positive. Pharmacists supported the utilization of ChatGPT to assist with tasks not directly related to complex clinical decision-making. Aligned with these positive perceptions, the practice of using ChatGPT in clinical pharmacy services was remarkably high at 57.9%. Regarding usage patterns, pharmacists tended to use ChatGPT more frequently (frequently; 43.9%) for educational tasks and retrieving drug or disease information. Conversely, the technology was not routinely used (sometimes) for drug interaction identification (33.3%) and adverse drug reactions (33.3%), and was rarely used (rarely; 42.1%) for dosage adjustments due to concerns over clinical accuracy and data privacy. As an anticipatory measure, pharmacists consistently (always; 49.1%) cross-verified the information generated by ChatGPT with trusted clinical reference sources. No significant relationship was found between perceptions and the practice of using ChatGPT (p=0.954). Regarding sociodemographic characteristics, the level of education was significantly associated with pharmacists' perceptions of clinical pharmacy services (p=0.025), and prior usage experience was significantly correlated with perceptions of ChatGPT as an emerging technology (p=0.005).
Conclusion: This study demonstrates that pharmacists generally accept and actively utilize ChatGPT, although they strictly restrict its application to educational and informative tasks due to underlying concerns regarding data privacy and clinical accuracy. Consequently, the development of official regulations and formal training on clinical risk mitigation are urgently required.
