Background: Diarrhea is one of the gastrointestinal complications in children with acute lymphoblastic leukemia (ALL) undergoing chemotherapy. Its occurrence is multifactorial and may be associated with chemotherapy-related toxicity, neutropenia, antibiotic use, and the phase of chemotherapy. Identification of predictors of diarrhea is important to improve clinical awareness and facilitate earlier detection and management in children with ALL.
Objective: To identify and evaluate high-dose methotrexate (HD-MTX) exposure, neutropenia, previous antibiotic use, and chemotherapy phase as predictors of diarrhea in children with ALL undergoing chemotherapy.
Methods: This was an analytical observational study with a matched case-control design using medical records of children with ALL undergoing chemotherapy at Sardjito Hospital from January 2019 to December 2023. Cases and controls were matched 1:1 based on age, sex, and ALL risk stratification. A total of 69 matched case-control pairs were analyzed using conditional logistic regression.
Results: Neutropenia was associated with diarrhea in the bivariate analysis (OR 2.09; 95% CI 1.02–4.29; p=0.044) and remained an independent predictor in the multivariable analysis (aOR 2.60; 95% CI 1.12–6.05; p=0.026). The induction phase was also independently associated with higher odds of diarrhea compared with the maintenance phase (aOR 3.62; 95% CI 1.11–11.76; p=0.032). HD-MTX exposure and previous antibiotic use were not significantly associated with diarrhea.
Conclusion: Neutropenia was an independent predictor of diarrhea in children with ALL undergoing chemotherapy. The induction phase was associated with higher odds of diarrhea compared with the maintenance phase after adjustment for other factors. HD-MTX exposure and previous antibiotic use were not demonstrated to be predictors of diarrhea in this study.
