Yogyakarta City faces population ageing, with older persons accounting for 16.13% of its population in 2025, far above the national figure. The Integrated Elderly Service (Layanan Lansia Terintegrasi, LLT) was developed as a policy innovation to reach highly dependent older persons through community-based case management at the kelurahan level. The spread of LLT from the pilot kelurahan to five other kelurahan produced uneven levels of adoption and development, even though all were facilitated by the same actor. This study analyses the mechanism of diffusion and adoption of the LLT policy innovation and explains how the innovation operates in Yogyakarta City.
A qualitative approach with a case study strategy was applied across the five implementing kelurahan: Gedongkiwo, Purbayan, Baciro, Kotabaru, and Semaki. Data were collected through in-depth interviews with 17 informants drawn from regional government agencies, kelurahan-level LLT management, and kelurahan officials, supplemented by policy documentation, and analysed using Rogers' (2003) diffusion of innovations framework.
The findings show that the diffusion of LLT unfolded as a gradual process not driven by formal mandate alone. Its establishment through a Mayoral Decree was followed by two phases of stagnation, and diffusion only gained momentum once formal legitimacy, technical capacity, and an external trigger became available in sequence. Information spread almost entirely through interpersonal communication, while acceptance of the innovation rested on the use of established social structures together with norms of respect for older persons, voluntarism, and religious values. The adoption process did not fully follow the linear sequence of Rogers' model: the persuasion and decision stages merged into a single moment of acceptance, the implementation stage was marked by re-invention, and confirmation took the form of additional innovations initiated by kelurahan managers themselves. Differences in development across kelurahan were explained neither by the order of designation, the order of mentoring, nor the number of older residents, but by the direction from which implementation was initiated and the composition of the management team formed at the kelurahan level. As an innovation, LLT is identified as a community-based social policy innovation of a coordinative character, whose novelty is integrative in service processes and methods, evident in cross-sectoral integration directed by Bappeda, the case management method, delivery within the community sphere, dual-node leadership, and the mobilisation of informal resources from local networks.
