Efektivitas Asetilsistein dan Ambroxol terhadap Luaran Klinis pada Pasien Penyakit Paru Obstruktif Kronis Pasca Eksaserbasi Akut

Latar Belakang: Batuk merupakan salah satu gejala yang sering menetap pada pasien penyakit paru obstruktif kronis (PPOK) setelah mengalami eksaserbasi akut. Asetilsistein dan ambroxol merupakan agen mukolitik yang umum digunakan dalam praktik klinis dengan mekanisme kerja dan biaya terapi yang berbeda. Namun, hasil studi yang membandingkan efektivitas kedua agen tersebut masih bervariasi. 

Tujuan: Membandingkan efektivitas asetilsistein dan ambroxol dalam menurunkan Cough Symptom Score (CSS) dan COPD Assessment Test (CAT) pada pasien PPOK pasca eksaserbasi akut. 

Metode: Penelitian dilakukan dengan rancangan Prospective Randomized Open Blinded Endpoint (PROBE) yang melibatkan pasien PPOK eksaserbasi akut berusia ? 40 tahun pasca rawat inap di RS Paru Respira Yogyakarta. Subjek dibagi menjadi dua kelompok secara acak yang menerima terapi tambahan asetilsistein (n=25) 200 mg tiap 8 jam atau ambroxol (n=25) 30 mg tiap 8 jam selama 14 hari. Luaran klinis yang diukur adalah kuesioner CSS, dinilai pada hari ke-0 dan hari ke-15 sesudah intervensi; skor CAT dinilai pada hari ke-0 dan hari ke-30 sesudah intervensi; serta perubahan (?) skornya. Analisis data menggunakan Wilcoxon-Signed Rank Test atau Paired T-test untuk perbandingan intrakelompok dan Mann-Whitney Test atau Independent sample T-test untuk perbandingan antar kelompok. Faktor-faktor yang memengaruhi hasil penelitian juga dianalisis menggunakan regresi linear.

Hasil: Hasil penelitian menunjukkan tidak terdapat perbedaan yang signifikan secara statistik antara kelompok asetilsistein dan ambroxol terhadap perubahan CSS siang (p=0,574), CSS malam (p=0,222), maupun skor CAT (p=0,274). Namun, analisis intrakelompok menunjukkan adanya penurunan skor yang signifikan antara sebelum dan sesudah intervensi pada seluruh luaran klinis (p<0 p=0,012), class="Apple-converted-space"> 

Kesimpulan: Dengan demikian, asetilsistein dan ambroxol memiliki efektivitas yang relatif setara dalam memperbaiki gejala batuk dan kualitas hidup pasien PPOK pasca eksaserbasi akut.

 Introduction: Cough is one of the symptoms that frequently persists in patient with Chronic Obstructive Pulmonary Disease (COPD) following an acute exacerbation. Acetylcysteine and ambroxol are commonly used mucolytic agents in clinical practice, with different mechanisms of action and treatment costs. However, studies comparing the effectiveness of these two agents have reported mixed results.

Objectives: Compare the effectiveness of acetylcysteine and ambroxol in reducing Cough Symptom Score (CSS) and COPD Assessment Test (CAT) in COPD patients after an acute exacerbation.

Method: This study used a Prospective Randomized Open Blinded Endpoint (PROBE) design, involving patients with acute exacerbations of COPD aged ?40 years after hospitalization at Respira Lung Hospital Yogyakarta. Subjects were randomly assigned into two groups, receiving additional therapy with acetylcysteine (n=25) 200 mg every 8 hours or ambroxol (n=25) 30 mg every 8 hours for 14 days. The clinical outcomes evaluated were CSS, measured at baseline (Day 0) and Day 15 post-intervention; CAT score, assessed at baseline and on Day 30 after intervention; as well as their changes (?). Data were analyzed using the Wilcoxon Signed-Rank Test or Paired T-test for within-group comparisons, and the Mann-Whitney Test or Independent T-test for between-group comparisons. Factors influencing the study were also analyzed using linear regression.

Result: The results showed no statistically significant differences between acetylcysteine and ambroxol groups in terms of changes in daytime CSS (p=0.574), nighttime CSS (p=0.222), or CAT score (p=0.274). However, within-group analysis demonstrated significant reductions in scores between pre- and post- intervention for all clinical outcomes (p<0 p=0.012), class="Apple-converted-space"> 

Conclusion: Thus, acetylcysteine and ambroxol demonstrate comparable effectiveness in improving cough symptoms and quality of life in patients with COPD following acute exacerbation.
