Efektivitas Triheksifenidil Pada Sindrom Ekstrapiramidal Akibat Antipsikotik Pada Pasien Skizofrenia

One of the side effects of antipsychotic use is extrapyramidal syndrome (EPS). EPS can be managed using anticholinergic drugs such as trihexyphenidyl for the treatment and prevention of antipsychotic-induced extrapyramidal syndrome. The use of trihexyphenidyl is expected to be effective in managing EPS side effects in patients, thereby improving patient adherence to treatment. This study aims to analyze the most dominant EPS symptoms in patients with schizophrenia and to evaluate the effectiveness of trihexyphenidyl in managing antipsychotic-induced extrapyramidal syndrome in patients with schizophrenia.
This study is an observational study with a cross-sectional design. Data collection was conducted retrospectively on adult patients using antipsychotics and trihexyphenidyl with a diagnosis of schizophrenia who showed EPS symptoms and underwent outpatient therapy at the Grahsia Mental Hospital in Yogyakarta from January 2024 to December 2024. Sampling used the consecutive sampling technique. Effectiveness was identified based on the diagnosis by the Patient's Responsible Physician (DPJP) documented in the medical record. Statistical analysis used included chi-square, binary logistic regression, and Kaplan Meire.
A total of 85 patients who met the inclusion criteria were included as subjects. This study identified seven EPS symptoms based on the diagnosis of the attending psychiatrist. The most dominant extrapyramidal symptom was tremor, experienced by 69 patients (81.18%), followed by rigidity in 13 patients (15.29%), and involuntary movements of the tongue, jaw, lips, or face in 13 patients (15.29%). The conclusion of this study is that trihexyphenidyl is effective in reducing extrapyramidal symptoms, with improvement observed in 75.4% of patients with tremor, 69.3% of patients with rigidity, and 69.3% of patients with involuntary movements of the tongue, jaw, lips, or face.
