Background: Health services still do not understand how much it costs to provide services to patients and how these costs compare with the results obtained from payers or health insurance providers. Cost analysis was carried out as an effort to evaluate the health service costs that are a burden on hospitals for urinary tract stone lithotripsy services with the Extracorporeal Shock Wave Lithotripay (ESWL) procedure carried out on BPJS Health patients. The cost analysis method used is Time Driven Activity Based Costing (TDABC) which calculates the unit cost value of the service, then compares it with the relative consequences received from BPJS Health as a health insurance provider, whether over value or under value.
Objective: Analysis of health service costs using the Time Driven Activity Based Costing (TDABC) method aims to obtain the unit cost value of the activities and time required for a series of urinary tract stone lithotripsy services using the Extracorporeal Shock Wave Lithotripsy (ESWL) procedure compared with the rates. INA CBG's paid by BPJS Health is over value or under value.
Method: This research is an analytical observational study with a case control design that identifies independent and dependent variable data based on criteria that have been determined in cases that have occurred. The independent variable data in this study are the activities and time required for the series of Urinary Tract Stone lithotripsy services using the Extracorporeal Shock Wave Lithotripsy (ESWL) procedure which were obtained through an interview mechanism with the personnel involved. Data on the dependent variable is secondary data from the Finance and Accounting Department of Siti Khodijah Hospital Pekalongan as a basis for determining Direct Cost and Indirect Cost allocations.
Results: The number of ESWL procedures for Urinary Tract Stone patients at Siti Khodijah Hospital Pekalongan during 2022 was 87 procedures and in 2023 was 76 procedures. Of the number of procedures performed, there were patients who received more than 1 procedure. The unit cost value per 1 series of ESWL procedures is IDR 3,970,423. The unit cost of the series of service activities is the unit cost generated in service activities in episode 1 of IDR 195,312 and the unit cost generated in service activities in episode 2 of IDR 3,775,111. INA CBG's rates are reviewed from PMK 52/2016 and PMK 3/2023. INA CBG's rates in Episode 1 based on PMK 52/2016 are IDR 190,400 and IDR 196,100 based on PMK 3/2023. INA CBG's Episode 2 rates based on PMK 52/2016 are IDR 4,126,200 and IDR 4,332,700 based on PMK 3/2023.
Conclusion: Unit cost of service activity before Lithotripsy Urinary Tract Stone ESWL method in episode 1 was Rp 195,312,- over value of INA CBG's Tariff based on PMK 52 / 2016 Rp 190,400,- and under value based on PMK 3 / 2023 Rp 196,100,-. Unit cost of service activity of Lithotripsy Urinary Tract Stone ESWL procedure in episode 2 was Rp 3,775,111,- under value of INA CBG's Tariff based on PMK 52 / 2016 Rp 4,126,200,- and based on PMK 3 / 2023 Rp 4,332,700,-. Meanwhile, the unit cost of a series of service activities for the Lithotripsy Action for Urinary Tract Stones ESWL procedure with a total of IDR 3,970,423 is below the value of the INA CBG's Tariff based on PMK 52/2016 of IDR 4,316,600 and based on PMK 3/2023 of IDR 4,528,800,-.
