Analisis Biaya Layanan Kesehatan dengan Metode Time Driven Activity Based Costing (TDABC) : Studi pada Tindakan Lithoripsy Batu Saluran Kemih Prosedur Extracorporeal Shock Wave Lithotripsy (ESWL) di Rumah Sakit Siti Khodijah Pekalongan

Latar Belakang: Pelayanan kesehatan masih belum memahami berapa biaya yang harus dikeluarkan untuk memberikan layanan kepada pasien dan bagaimana biaya tersebut dibandingkan dengan hasil yang diperoleh dari pembayar atau provider jaminan kesehatan. Analisis biaya dilakukan sebagai upaya evaluasi biaya layanan kesehatan yang menjadi beban rumah sakit pada layanan tindakan lithotripsy batu saluran kemih dengan prosedur Extracorporeal Shock Wave Lithotripay (ESWL) yang dilakukan kepada pasien BPJS Kesehatan. Metode analisis biaya yang digunakan adalah Time Driven Activity Based Costing (TDABC) yang menghitung nilai unit cost pelayanan tersebut, kemudian membandingkannya dengan konsekuensi relatif yang diterima dari BPJS Kesehatan sebagai provider jaminan kesehatan apakah over value atau under value.

Tujuan: Analisis biaya layanan kesehatan dengan menggunakan metode Time Driven Activiy Based Costing (TDABC) bertujuan untuk mendapatkan nilai unit cost dari aktivitas dan waktu yang dibutuhkan pada rangkaian pelayanan tindakan lithotripsy batu saluran kemih dengan prosedur Extracorporeal Shock Wave Lithotripsy (ESWL) yang dibandingkan dengan Tarif INA CBG’s yang dibayarkan oleh BPJS Kesehatan apakah over value atau under value.

Metode: Penelitian ini merupakan penelitian observasional analitik dengan desain case control yang mengidentifikasi data variabel bebas dan variabel terikat berdasarkan kriteria yang telah ditetapkan pada kasus yang telah terjadi. Data variabel bebas dalam penelitian ini adalah aktivitas dan waktu yang dibutuhkan pada rangkaian pelayanan tindakan lithotripsy Batu Saluran Kemih dengan Prosedur Extracorporeal Shock Wave Lithotripsy (ESWL) yang diperoleh melalui mekanisme wawancara kepada personil yang terlibat. Data pada variabel terikat merupakan data sekunder dari Departemen Keuangan dan Akuntansi Rumah Sakit Siti Khodijah Pekalongan sebagai dasar penetapan alokasi Direct Cost dan Indirect Cost.

Hasil : Jumlah tindakan ESWL pada pasien Batu Saluran Kemih di Rumah Sakit Siti Khodijah Pekalongan selama tahun 2022 adalah 87 tindakan dan pada tahun 2023 adalah 76 tindakan. Dari jumlah tindakan yang dilakukan tersebut, terdapat pasien yang mendapatkan tindakan lebih dari 1 kali. Nilai unit cost per 1 kali rangkaian aktivitas tindakan ESWL adalah Rp 3.970.423,-. Unit Cost rangkaian aktivitas pelayanan tersebut merupakan unit cost yang dihasilkan pada aktivitas pelayanan di episode 1 sebesar Rp 195.312,- dan unit cost yang dihasilkan pada aktivitas pelayanan di episode 2 sebesar Rp 3.775.111,-. Tarif INA CBG’s ditinjau dari PMK 52 / 2016 dan PMK 3 / 2023. Tarif INA CBG’s pada Episode 1 berdasarkan PMK 52 / 2016 sebesar Rp 190.400,- dan Rp 196.100 berdasarkan PMK 3 / 2023. Tarif INA CBG’s Episode 2 berdasarkan PMK 52 / 2016 sebesar Rp 4.126.200 dan Rp 4.332.700 berdasarkan PMK 3 / 2023.

Kesimpulan : Unit cost aktivitas pelayanan pra Tindakan Lithotripsy Batu Saluran Kemih metode ESWL pada episode 1 sebesar Rp 195.312,- over value terhadap Tarif INA CBG’s berdasarkan PMK 52 / 2016 Rp 190.400,- dan under value berdasarkan PMK 3 / 2023 Rp 196.100,-. Unit cost aktivitas pelayanan Tindakan  Lithotripsy Batu Saluran Kemih prosedur ESWL pada episode 2 sebesar Rp 3.775.111,- under value terhadap Tarif INA CBG’s berdasarkan PMK 52 / 2016 Rp 4.126.200,- dan berdasarkan PMK 3 / 2023 Rp 4.332.700,-. Sedangkan Unit cost rangkaian aktivitas pelayanan Tindakan Lithotripsy Batu Saluran Kemih prosedur ESWL dengan total sebesar Rp 3.970.423,- under value terhadap Tarif INA CBG’s berdasarkan PMK 52 / 2016 Rp 4.316.600,- dan berdasarkan PMK 3 / 2023 Rp 4.528.800,-.

Background: Health services still do not understand how much it costs to provide services to patients and how these costs compare with the results obtained from payers or health insurance providers. Cost analysis was carried out as an effort to evaluate the health service costs that are a burden on hospitals for urinary tract stone lithotripsy services with the Extracorporeal Shock Wave Lithotripay (ESWL) procedure carried out on BPJS Health patients. The cost analysis method used is Time Driven Activity Based Costing (TDABC) which calculates the unit cost value of the service, then compares it with the relative consequences received from BPJS Health as a health insurance provider, whether over value or under value.

Objective: Analysis of health service costs using the Time Driven Activity Based Costing (TDABC) method aims to obtain the unit cost value of the activities and time required for a series of urinary tract stone lithotripsy services using the Extracorporeal Shock Wave Lithotripsy (ESWL) procedure compared with the rates. INA CBG's paid by BPJS Health is over value or under value.

Method: This research is an analytical observational study with a case control design that identifies independent and dependent variable data based on criteria that have been determined in cases that have occurred. The independent variable data in this study are the activities and time required for the series of Urinary Tract Stone lithotripsy services using the Extracorporeal Shock Wave Lithotripsy (ESWL) procedure which were obtained through an interview mechanism with the personnel involved. Data on the dependent variable is secondary data from the Finance and Accounting Department of Siti Khodijah Hospital Pekalongan as a basis for determining Direct Cost and Indirect Cost allocations.

Results: The number of ESWL procedures for Urinary Tract Stone patients at Siti Khodijah Hospital Pekalongan during 2022 was 87 procedures and in 2023 was 76 procedures. Of the number of procedures performed, there were patients who received more than 1 procedure. The unit cost value per 1 series of ESWL procedures is IDR 3,970,423. The unit cost of the series of service activities is the unit cost generated in service activities in episode 1 of IDR 195,312 and the unit cost generated in service activities in episode 2 of IDR 3,775,111. INA CBG's rates are reviewed from PMK 52/2016 and PMK 3/2023. INA CBG's rates in Episode 1 based on PMK 52/2016 are IDR 190,400 and IDR 196,100 based on PMK 3/2023. INA CBG's Episode 2 rates based on PMK 52/2016 are IDR 4,126,200 and IDR 4,332,700 based on PMK 3/2023.

Conclusion: Unit cost of service activity before Lithotripsy Urinary Tract Stone ESWL method in episode 1 was Rp 195,312,- over value of INA CBG's Tariff based on PMK 52 / 2016 Rp 190,400,- and under value based on PMK 3 / 2023 Rp 196,100,-. Unit cost of service activity of Lithotripsy Urinary Tract Stone ESWL procedure in episode 2 was Rp 3,775,111,- under value of INA CBG's Tariff based on PMK 52 / 2016 Rp 4,126,200,- and based on PMK 3 / 2023 Rp 4,332,700,-. Meanwhile, the unit cost of a series of service activities for the Lithotripsy Action for Urinary Tract Stones ESWL procedure with a total of IDR 3,970,423 is below the value of the INA CBG's Tariff based on PMK 52/2016 of IDR 4,316,600 and based on PMK 3/2023 of IDR 4,528,800,-.
