Analisis Dampak Kebijakan Pelayanan Gigi Spesialistik Peserta JKN Terhadap Kunjungan Rawat Jalan Di Rumah Sakit Wilayah Kota Depok

Latar belakang: Dalam beberapa regulasi terkait Program Jaminan Kesehatan Nasional (JKN) yaitu Peraturan Presiden Nomor 82 Tahun 2018 disebutkan bahwa Fasilitas Kesehatan Rujukan Tingkat Lanjut (FKRTL) adalah fasilitas kesehatan yang melakukan pelayanan kesehatan perorangan yang bersifat spesialistik atau sub spesialistik, dan pada Peraturan Menteri Kesehatan No. 28 tahun 2014 tentang Pedoman Pelaksanaan Program JKN disebutkan bahwa manfaat pelayanan di FKRTL meliputi pemeriksaan, pengobatan dan konsultasi oleh dokter spesialis dan subspesialis. Untuk itu BPJS Kesehatan Cabang Depok sejak April 2021 menerapkan kebijakan pelayanan gigi bagi peserta JKN yang dapat diklaimkan oleh Rumah Sakit di Wilayah Kota Depok adalah pelayanan gigi oleh dokter gigi spesialis atau subspesialis. Penelitian ini dilakukan untuk mengeksplorasi dampak atas penerapan kebijakan pelayanan gigi spesialistik Peserta JKN terhadap kunjungan rawat jalan di Rumah Sakit Wilayah Kota Depok.

Metode: Penelitian mixed method kuantitatif dan kualitatif dengan desain quasi eksperimental. Teknik pengumpulan data menggunakan metode purposive sampling dari data sekunder BPJS Kesehatan Cabang Depok dan wawancara mendalam kepada manajemen Rumah Sakit. Subjek penelitian adalah seluruh Rumah Sakit di Wilayah Kota Depok yang bekerjasama dengan BPJS Kesehatan dalam memberikan pelayanan gigi kepada peserta JKN.

Hasil: Penerapan kebijakan pelayanan gigi spesialistik bagi peserta JKN di Rumah Sakit Wilayah Depok berdampak terhadap penurunan jumlah kunjungan dan biaya klaim rawat jalan. Setelah penerapan kebijakan tidak terdapat perbedaan pola diagnosis dan prosedur gigi. Berbagai upaya yang telah dilakukan Rumah Sakit dalam menyikapi kebijakan pelayanan gigi spesialistik bagi Peserta JKN yaitu menambah ketersediaan dokter gigi spesialis, penambahan frekuensi dan jam praktek dokter gigi spesialis, penatalaksanaan pasien secara lebih efektif oleh dokter gigi spesialis, transparan dalam negosisasi kesepakatan tarif jasa pelayanan dengan dokter gigi spesialis, relokasi penempatan dokter gigi spesialis khususnya bagi yang berstatus PNS, serta mendorong dokter gigi umum yang berpraktek di Rumah Sakit untuk meningkatkan kompetensi akademiknya menjadi dokter gigi spesialis.  

Kesimpulan: Penerapan kebijakan pelayanan gigi spesialistik bagi peserta JKN di Rumah Sakit Wilayah Depok berdampak terhadap penurunan jumlah kunjungan dan biaya klaim rawat jalan. Rumah Sakit telah menerapkan berbagai strategi dalam menyikapi kebijakan pelayanan gigi spesialistik bagi Peserta JKN dari aspek ketersediaan dokter gigi spesialis maupun aspek pelayanan.

Background: In several regulations related to the National Health Insurance (JKN) Program, namely Presidential Regulation Number 82 of 2018 stated that the Advanced Referral Health Facility (FKRTL) is a health facility that performs individual health services that are specialistic or sub-specialistic in nature, and in the Minister of Health Regulation No. 28 of 2014 concerning Guidelines for Implementing the JKN Program stated that the benefits of services at FKRTL include examination, treatment, and consultation by specialists and subspecialists. For this reason, BPJS Kesehatan Depok Branch, since April 2021, has implemented a dental service policy for JKN participants that can be claimed by hospitals in the Depok Region, namely dental services by specialist or subspecialist dentists. This study was conducted to explore the impact of the implementation of the JKN Participants' specialty dental service policy on outpatient visits at the Depok Region Hospitals.

Methods: The study used mixed method quantitative and qualitative research with a quasi-experimental design. Data collection techniques used a purposive sampling method from secondary data obtained from BPJS Kesehatan Depok Branch and in-depth interviews with hospital management. The research subjects were all hospitals in the Depok Region that collaborated with BPJS Kesehatan in providing dental services to JKN participants.

Results: Implementing the specialty dental service policy for JKN participants at the Depok Regional Hospitals has had impacts decreasing the number of outpatient visits and outpatient claim costs. After policy implementation, there were no differences in diagnosis patterns and dental procedures. Various efforts made by the hospital in responding to the specialty dental service policy for JKN Participants include increasing the availability of specialist dentists, increasing the frequency and practice hours of specialist dentists, more effective patient management by specialist dentists, transparency in negotiating agreements for specialist dentist service rates, relocation of specialist dentist placements, especially for those with Civil Servant (PNS) status, and encouraging the increase in the competence of general dentists practicing in hospitals to become specialist dentists.  

Conclusion: The specialty dental service policy for JKN participants in Depok Region Hospitals has had impacts decreasing the number of visits and outpatient claim costs. Hospitals have implemented various strategies in responding to the specialty dental service policy for JKN participants from the aspect of the availability of specialist dentists and service aspects.
