Analisis Determinan Pemanfaatan Pelayanan Kesehatan di Fasilitas Kesehatan Tingkat Pertama Pada Peserta Jaminan Kesehatan Nasional

Pemerintah Indonesia telah mencanangkan UHC pada 1 Januari 2014 melalui program Jaminan Kesehatan Nasional (JKN) dan dikelola oleh Badan Penyelenggaran Jaminan Sosial (BPJS) Kesehatan. Tujuan penelitian ini adalah mengetahui gambaran pemanfaatan pelayanan kesehatan oleh peserta JKN dan menganalisis determinan yang berpengaruh dalam pemanfaatan pelayanan kesehatan oleh peserta JKN di fasilitas kesehatan tingkat pertama.

Penelitian ini merupakan penelitian observasional analitik dengan pengumpulan data secara cross-sectional menggunakan data berbasis survei populasi. Responden dari penelitian ini adalah peserta JKN yang memanfaatkan fasilitas kesehatan tingkat pertama. Jumlah responden sebesar 390 peserta dengan pengumpulan data melalui kuesioner survei berbasis online. Analisis yang dilakukan adalah analisis deskriptif dan analisis bivariat dengan uji chi-square.

Hasil dari penelitian sebanyak 293 (75,1%) responden memanfaatkan pelayanan kesehatan ketika sakit. Status pernikahan (p=0,019), tingkat pendidikan (p=0,049), jumlah penghasilan (p=0,000), dan jenis asuransi kesehatan yang dimiliki (p=0,000) berhubungan terhadap pemanfaatan pelayanan kesehatan. Jumlah penghasilan (p=0,013) dan kemudahan akses (p=0,008) berhubungan terhadap pemilihan fasilitas kesehatan. Tingkat pendidikan (p=0,038) berhubungan terhadap pemilihan metode pembayaran. Diperlukan peningkatan kualitas pelayanan kesehatan pada peserta Jaminan Kesehatan Nasional (JKN) khususnya di fasilitas tingkat pertama dengan mempertimbangkan kemudahan akses dan keterjangkauan layanan kesehatan. Penelitian selanjutnya diharapakan dapat meneliti faktor-faktor lain yang dapat lebih mendalami topik penelitian ini.

The Indonesian government launched UHC on January 1, 2014, through the National Health Insurance (JKN) program and is managed by the Health Social Security Administration Agency (BPJS). The purpose of this study was to describe the utilization of health services by JKN participants and to analyze the determinants that influence the utilization of health services by JKN participants in first-level health facilities.

This research is an analytic observational study with cross-sectional data collection using data based on population surveys. Respondents from this study were JKN participants who utilized first-level health facilities. The number of respondents was 390 participants with data collected through online-based survey questionnaires. The analysis used was descriptive analysis and bivariate analysis using the chi-square test.

The results of the study were 293 (75.1%) respondents used health services when they were sick. Marital status (p=0.019), level of education (p=0.049), total income (p=0.000), and type of health insurance (p=0.000) are related to the utilization of health services. Total income (p=0.013) and ease of access (p=0.008) are related to the selection of health facilities. Education level (p=0.038) is related to the choice of payment method. It is necessary to improve the quality of health services for National Health Insurance (JKN) participants, especially at first-level facilities by considering the ease of access and affordability of health services. Future research is expected to be able to examine other factors that can further explore the topic of this research.
