Gambaran Drug-Related Problems (DRPs) Penggunaan Antibiotik pada Pasien Pediatri serta Peran Apoteker dalam Mengatasinya di Puskesmas Wilayah Kota Yogyakarta: Narrative Review dan Penelitian Kualitatif

Antibiotik merupakan obat yang sering diresepkan, namun penggunaannya masih kurang rasional. Berbagai permasalahan berkaitan dengan Drug-Related Problems (DRPs) sering dijumpai dalam pelayanan antibiotik. Tidak hanya pada orang dewasa, antibiotik juga ditemui pada peresepan pediatri. Penelitian ini bertujuan untuk mengetahui gambaran DRPs antibiotik pada pasien pediatri, serta peran apoteker dalam mengatasi DRPs yang terjadi. Penelitian ini adalah kombinasi narrative review dan penelitian kualitatif. Tahap pertama, yaitu narrative review dengan pencarian dan seleksi artikel sesuai kriteria inklusi dan eksklusi. Tahap kedua, yaitu penelitian kualitatif melalui wawancara terhadap responden yang ditentukan dengan purposive sampling. Responden adalah apoteker yang bekerja di puskesmas wilayah Kota Yogyakarta. Penelitian dilakukan di lima puskesmas Kota Yogyakarta dalam rentang waktu Januari-Juni 2022. Data yang didapatkan, dianalisis dan diolah menjadi data deskriptif yang menggambarkan kejadian DRPs penggunaan antibiotik pada pasien pediatri di dunia dan di Puskesmas Kota Yogyakarta, serta peran apoteker dalam mengatasi DRPs yang sering terjadi. Pada tahap pertama, yaitu narrative review diperoleh 17 artikel yang menggambarkan DRPs antibiotik untuk pediatri secara global. Kategori DRPs yang paling sering ditemukan, yaitu dosis tidak tepat, salah obat, dan reaksi obat merugikan. Pada tahap kedua, yaitu wawancara terhadap lima apoteker puskesmas wilayah Kota Yogyakarta didapatkan kategori DRPs yang sering ditemukan adalah dosis terlalu rendah, reaksi obat merugikan, dan kepatuhan. Rentang persentase kejadian DRPs antibiotik untuk pediatri berdasarkan pengalaman apoteker sebesar 1,34-40%. Peran yang dilakukan apoteker untuk mengatasi DRPs adalah pengkajian resep, penggalian informasi, konfirmasi kepada penulis resep, serta edukasi pasien.

This study aims to determine the description of antibiotic DRPs in pediatric patients, as well as the role of pharmacists in overcoming DRPs that occur. This research is a combination of narrative review and qualitative research. The first stage is a narrative review by searching and selecting articles according to the inclusion and exclusion criteria. The second stage, namely qualitative research through interviews with respondents determined by purposive sampling. Respondents are pharmacists who work in health centers in the city of Yogyakarta. The study was conducted in five health centers in Yogyakarta City in the period January-June 2022. The data obtained, analyzed and processed into descriptive data describes the incidence of DRPs using antibiotics in pediatric patients in the world and in Yogyakarta City Health Centers, as well as the role of pharmacists in overcoming DRPs that often occur. occur. In the first stage, namely a narrative review, 17 articles were obtained that describe antibiotic DRPs for pediatrics globally. The most frequently found categories of DRPs were inappropriate dose, wrong drug, and adverse drug reaction. In the second stage, namely interviews with five pharmacists in the Yogyakarta City Health Center, the categories of DRPs that were often found were too low doses, adverse drug reactions, and adherence. The percentage range of the incidence of antibiotic DRPs for pediatrics based on pharmacist experience is 1.34-40%. The role of pharmacists in overcoming DRPs is to review prescriptions, gather information, confirm with prescribers, and educate patients.
