ABSTRAK Latar Belakang: Puskesmas Gamping II Sleman Yogyakarta merupakan puskesmas yang telah menerapkan Mesin Anjungan Pedaftaran Mandiri (APM) untuk melakukan pendaftaran pasien. Penerapan mesin APM telah ada sejak bulan Januari 2019, sehingga mesin APM ini tergolong baru di Puseksmas Gamping II. Dalam penerapan mesin ini, belum pernah dilakukan evaluasi maupun penilaian terkait persepsi pengguna terhadap penerimaan teknologi tersebut. Tujuan: Mengevaluasi penerapan Mesin Anjungan Pendaftaran Mandiri (APM) pasien rawat jalan di Puskesmas Gamping II Sleman Yogyakarta. Metode Penelitian: Jenis Penelitian yang digunakan merupakan penelitian kuantitatif dengan rancangan cross sectional dan pendekatan analitik. Teknik pengambilan data menggunakan kuisioner dan sampel penelitian ini berjumlah 98 responden. Teknik analisis data menggunakan teknik analisis korelasi Spearman-rank dengan menggunakan Software R. Hasil: Variabel yang berpengaruh secara signifikan, yaitu: user training terhadap perceived ease of use dan perceived usefulness yang menunjukkan hasil p_value masing-masing 0,0001982 dan 0,00359 , perceived ease of use terhadap perceived usefulness hasil p_value 0,005577 , perceived ease of use dan perceived usefulness terhadap intention to use p_value masing-masing 0,004379 dan 0,001706. Kesimpulan: Terdapat 5 variabel yang berpengaruh signifikan yaitu: user training terhadap perceived ease of use, user training terhadap perceived usefulness, perceived ease of use terhadap perceived usefulness, perceived ease of use terhadap intention to use, perceived usefulness terhadap intention to use. Dan terdapat satu variabel yang tidak signifikan yaitu: intention to use terhadap actual system use. Kata kunci: Evaluasi, mesin anjungan pendaftaran mandiri, TAM.

ABSTRACT Backgrounds: Gamping II Sleman primary health care is pimary health care that applies Self-Registration Machine for regist patient. Self-Registration Machine has been implementated since Januari 2019, so that machine relativity new registration service at Gamping II Sleman primary health care. The implementation of this machine, there has never been an evaluation or assesment related to the users perception of the technology. Objective: To evaluate implementation Self-Registration Machine for outpatien at Puskesmas Gamping II Sleman. Methods: This research is quantitative with cross sectional design and analytical approach. The data collection technique used questionnaires and the subjects are 98 respodens. The data analytics technigue used Spearman-rank correlation analysis technique using R software. Result: The variables that have a significant effect, includes: user training to perceived ease of use and perceived usefulness that each p_value result are 0,0001982 and 0,00359, perceived ease of use to perceived usefulness that p_value is 0,005577, perceived ease of use and perceived usefulness to intention to use with p_value are 0,004379 and 0,001706. Conclusion: There are 5 variables that have a significant effect, includes: user training to perceived ease of use, user training to perceived usefulness, perceived ease of use to perceived usefulness, perceived ease of use to intention to use, perceived usefulness to intention to use.
