EFFECTIVENESS OF DIAGNOSIS-RELATED GROUP-BASED PAYMENT SYSTEM IN IMPROVING QUALITY OF CARE

Biaya perawatan kesehatan yang meningkat di banyak negara, memberikan tekanan besar pada pemerintah untuk mengendalikan biaya. Salah satu solusi terhadap masalah tersebut adalah dengan mereformasi sistem kompensasi para penyedia medis, dan menerapkan Diagnosis-Related Group-Based Payment System adalah alternatif yang paling sering diterapkan. Meskipun sistem pembayaran berbasis DRG berasal dari Amerika Serikat, keunggulannya telah menyebar ke benua lain, termasuk Eropa. Secara umum, tujuan dari sistem ini adalah efisiensi, tetapi tantangan utama dalam industri perawatan kesehatan adalah tidak hanya memberikan perawatan yang hemat biaya tetapi juga secara efektif, oleh karena itu kualitas perawatan sama pentingnya. Oleh karena itu, penelitian ini bertujuan untuk menganalisis apakah sistem ini juga efektif dalam meningkatkan kualitas perawatan, khususnya di Inggris Raya dan Jerman. Temuan dalam penelitian ini menyarankan bahwa sistem pembayaran berbasis DRG tidak berpengaruh pada kualitas perawatan di Inggris dan efek negatif di Jerman. Oleh karena itu, sistem pembayaran berbasis DRG bukanlah alat yang efektif untuk meningkatkan kualitas layanan, tidak hanya karena dampak negatifnya pada kualitas layanan, tetapi juga karena pada dasarnya, sistem tersebut tidak mendorong penyedia layanan kesehatan untuk meningkatkan kualitas layanan.

Health care costs are rising in many countries, putting considerable pressure on the government to control the costs. One possible solution is to reform the way medical providers are paid, and implementing Diagnosis-Related Group-Based payment system is the most widely used alternative in doing so. Even though the DRG-based payment system started out in the United States, its prominence has spread out to other continents, including Europe. Generally, the aim of the system is efficiency, but the main challenge in the health care industry is not only to deliver treatments cost-efficiently but also effectively, therefore the quality of care is equally imperative. Therefore, this paper aims to analyze whether the system is also effective in improving the quality of care, specifically in the United Kingdom and Germany. The findings in this paper suggested that the DRG-based payment system has no effect on the quality of care in the United Kingdom and a negative effect in Germany. Therefore, DRG-based payment system is not an effective tool to improve the quality of care, not only because the negative impact it has on the quality of care, but also because fundamentally, the system does not encourage medical providers to improve the quality in the first place
