PERBANDINGAN TEKANAN KEBOCORAN OROFARING PADA PEMAKAIAN LMA PROSEAL ANTARA POSISI KEP AlA DAN LEHER NETRAL DENGAN ROTASI KANAN MAKSIMAL

Latar belakang. Saat ini sungkup laring (LMA) sudah dipergunakan dalam manajemen anestesi pada operasi-operasi yang memerlukan manipulasi kepala dan Ieber. Secara ideal, LMA haruslah mempunyai tekanan kebocoran orofaring (oropharyllgeal leak pressure) yang cukup terhadap ventilasi tekanan positif terutama pada perubahan posisi kepala dan leher. LMA Proscal adalah LMA dengan modifikasi balon (cuff) yang didesain untuk memperbaiki penutup disekitar glotis dengan tambahan pipa drainasc untuk mcmasukkan selang lambung. Terdapat beberapa bukti yang menyatakan bahwa llcksi dan rotasi kepala leher berkaitan dengan peningkatan tekanan kebocoran orofaring pada LMA Proseal. Penelitian ini membandingkan tekanan kebocoran orofaring antam posisi kepala dan leher netral dengan rotasi kanan maksimal pada pemakaian L 11 A ProseaI. Metode. Setelah mendaratkan persetujuan komite etik dan persetujuan tindakan medis penderita. 40 pasien (I R-65 tahun, ASA 1- 11) yang menjalani pemhedahan terencana selain prosedur daerah kepala dan leher dimasukkan dalam uj i klinis aeak meny ilang tersamar ganda dan dibagi baik kedalam kelompok posisi kepala leher netral maupun rotasi kanan maksimal. Luaran primer adalah perbandingan tekanan kebocoran orofaring antara posisi kepala dan leher netral dengan rotasi kanan maksimal dan luaran sekunder ada lah perbandingan antara ukuran l ~1 \ Procaltcrhaclap tekanan kebocoran orofaring pada kc-2 posisi kepala dan leher. Hasil. tekanan kebocoran orofaring. antara posisi kepala dan leher netral dengan rota-.i kanan mab.imal bcrbeda bcrmakna cara statistik (P 0.0-t~). alaupun secant klinis tidalk didapatkan perbedaan dimana rerata tekanan kebocoran orofaring pada posisi kepala dan leher netraI dengan rotasi kanan maksimaI adalah 2.6.9.14,5 em 1120 vs 26.05±4,6 em I hO. Adapun tekanan kebocoran pada masing-masing ukuran LMA Proscalt idak berbeda bermakna pada posisi kepala dan leher netral (P- 0.565) maupun rota si kanan maksinwl (P=O, 11 6). Kesimpulan. Ada perbedaan tekanan kebocoran orofaring antara posisi kepala dan leher netral dengan rotasi kanan mabimal pada pemakaian LMA Pro..,cal (P 0,042). Backgr ound. LarynKea/ Mask Airway (LMA) has been used for the anesthesia that required various head and neck position. Ideally, the optimal LMA hould have an oropharyngc..1l leak pressure sufficient for positive pres ure ventilation even '' ith difrcrcnt head and neck positions. Proseal LMA is a LMA with a modi lied culT designed to improve the cal around the glottis '' ith in added draining tube for gastric tube placement.
