ANALISIS FAKTOR PENYEBAB TERJADINYA MISFILE PADA PASIEN POST RAWAT INAP DI RSUD WONOSARI

Semua pelayanan kesehatan di rumah sakit harus tertulis dalam rekam medis. Selain untuk bukti hukum, rekam medis digunakan untuk menjaga kesinambungan pelayanan kesehatan pasien. Di RSUD Wonosari, penyediaan berkas rekam medis tidak bisa dilakukan dengan cepat karena banyak terjadi misfile berkas rekam medis. Penelitikan ini dilakukan untuk melakukan analisis faktor penyebab terjadinya misfile pada pasien post rawat inap DI RSUD Wonosari. Tujuan:mengetahui persentase terjadinya misfile dan persentase tempat keberadaan berkas rekam medis post rawat inap, menganalisis faktor penyebab misfile pada pasien post rawat inap DI RSUD Wonosari. Metode: Jenis penelitian yang digunakan adalah penelitian deskriptif dengan pendekatan kualitatif dan rancangan penelitian fenomenologi. Subjek penelitian adalah 1 orang petugas pengolahan, 3 orang petugas filing dan 12 perawat. Objek penelitian adalah berkas rekam medis post rawat inap. Hasil:Selama 2 hari observasi, diketahui persentase terjadinya misfile di RSUD Wonosari sebesar 67%, berkas berada di rak penyimpanan sebesar 33%, di tempat pengolahan sebesar 47%, di bangsal sebesar 8%, dan 10% berkas yang tidak ditemukan. Faktor penyebab misfile berkas rekam medis post rawat inap terkait man adalah kurangnya pelatihan untuk petugas pengolahan berkas rawat inap dan petugas filing serta kurang pengetahuan perawat tentang maksimal waktu pengembalian berkas. Faktor penyebab terkait materials adalah kurangnya rak penyimpanan berkas rekam medis. Faktor penyebabab terkait method tidak tertatanya berkas sesuai dengan sistem penjajaran yang digunakan.

Every health service in the hospital must be noted into medical records. Aside from a legal proof, medical records are used to keep the continuity of patients' health services. In RSUD Wonosari, the provision of medical records' files could not be offered as soon as posible due to the misfiled of medical records occured. This research is aimed to analyse the causative factors of misfile on RSUD Wonosari's inpatient. Goal: To know the percentage of misfile and its placement on inpatents' medical records, to analyse the causative factors of misfile on RSUD Wonosari's inpatient. Metodhology: The research uses descriptive analysis with qualitative approach and fenomenology research design. The subject of the study is a medical record processing officer, three data filling officers, and 12 nurses. The object of the study is inpatients' medical records. Outcome: After 2 days of observation, it is found that the percentage of misfile in RSUD Wonosari is 67%, files on shelf storage is 33% , files in the analysis post is 47% , files in the ward is 8% , and 10% files are missing. The factors that cause misfile in related to the human resources is trainning for medical record processing officer and medical record filing officer, and also the ignorance of the due date for the files return. The factors that cause misfile in related to the material is inadequate shelves availability. The factors that cause misfile in related to the method is the arrangement of medical records' files which are not applied to all files.
