Tinjauan Pelaksanaan Sistem Identifikasi Pasien Rawat Jalan Berdasarkan SIMRS di Rumah Sakit Khusus Bedah Islam Cawas

Latar Belakang: Identifikasi pasien digunakan untuk memastikan perawatan yang benar untuk pasien yang benar. Hal ini merupakan bagian penting dalam menyediakan keamanan dan keselamatan pasien. Berdasarkan hasil studi pendahuluan terdapat beberapa masalah yaitu kesalahan pengambilan berkas akibat kesalahan identifikasi karena nama pasien sama dan terjadi perubahan SIMRS lama ke SIMRS baru bulan Januari 2016. Tujuan: Mengetahui pelaksanaan sistem identifikasi pasien rawat jalan menggunakan SIMRS, mengidentifikasi kendala terkait proses identifikasi pasien dan mengetahui upaya yang telah dilakukan akibat kesalahan identifikasi pasien. Metode: Jenis penelitian yang digunakan adalah penelitian secara deskriptif dengan pendekatan kualitatif dengan rancangan fenomenologi. Pengambilan data dilakukan secara studi dokumentasi, wawancara dan observasi. Subjek penelitian ini adalah 5 orang petugas pendaftaran dan 1 Kepala Sub Bagian Rekam Medis sebagai triangulasi. Objek penelitian ini adalah standar prosedur operasional (SPO) identifikasi pasien. Penelitian ini dilakukan di bagian pendaftaran Rumah Sakit Khusus Bedah Islam Cawas. Hasil: Pelaksanaan identifikasi pasien sudah dilakukan secara komputerisasi. Kendala dalam mengidentifikasi pasien adalah pasien lama tidak membawa kartu berobat, SIMRS error, pasien baru mengisi data di formulir tidak lengkap, petugas pendaftaran kurang teliti dan SPO tidak sesuai. Upaya yang dilakukan petugas adalah petugas melakukan cross check di SIMRS, petugas melakukan revisi SOP dan kebijakan berstandar akreditasi 2012. Kesimpulan: Identifikasi Pasien menggunakan SIMRS untuk memudahkan dalam pencarian data. Namun SIMRS error sehingga menjadi salah satu kendala dalam mengidentifikasi pasien. Saran yang diberikan peneliti adalah dilakukan evaluasi dan monitoring terhadap pelaksanaan identifikasi pasien.

Background: The identification of patients is used to ensuring the right care to the correct patient. This is an important part of providing security and safety of patients. Based on preliminary study there are some problems, retrieval error due to patient misidentification due to the same name patient and there are changes from the old SIMRS to new SIMRS since January 2016. Objective: Know the implementation of an identification system outpatients using SIMRS, identify constraints associated patient identification process and know the efforts that have been made due to patient identification errors. Methods: This type of research is descriptive study with a qualitative approach with the phenomenology design. Data were collected in the study documentation, interviews and observation. The subjects were 5 officers patient admission and one sub-section head of medical record as triangulation. The object of this study is the standard procedur operating (SPO) patient identification. This research was conducted in the patient admission Rumah Sakit Khusus Bedah Islam Cawas. Results: Implementation of patient identification already is computerized. Difficulties in identifying the patients are old patients did not bring medical card, SIMRS error, new patients filled out the form is not complete, the officer did not accurately and SPO are not appropriate. Efforts are made officer is crosscheck in SIMRS, officer revised SPO and policy standards of accreditation in 2012. Conclusions: Identification of patient using SIMRS to facilitate the search data. However SIMRS error that one of the obstacles in identifying patients. The advice given by researchers is the evaluation and monitoring of the implementation of patient identification.
