AKURASI NILAI DIFUSI APPARENT DIFFUSION COEFFICIENT MAPS INTRATUMORAL MAGNETIC RESONANCE IMAGING DALAM MENENTUKAN DERAJAT KEGANASAN ASTROSITOMA

Latar Belakang: Astrositoma adalah tumor otak primer yang berasal dari sel astrosit. Astrositoma menyebabkan > 80% tumor otak dengan tingkat mortalitas tinggi kedua sehingga diagnosis haris ditegakkan sedini mungkin. Astrositoma tediri dari tiga bagian yaitu keseluruhan bagian tumor, bagian padat tumor dan edema peritumoral. Pengukuran akurasi nilai difusi Apparent Diffusion Coefficient (ADC) maps intratumoral pada pencitraan MRI dapat menentukan derajat keganasan astrositoma. Pemeriksaan ini bersifat non-invasif dibandingkan dengan pemeriksaan histopatologi dalam menentukan derajat astrositoma. Tujuan: Adapun tujuan dari penelitian ini untuk mengetahui akurasi nilai difusi ADC maps intratumoral pada pencitraan MRI kepala kontras dibandingkan dengan baku emas Patologi Anatomi dalam penentuan derajat astrositoma. Metode. Penelitian observasional analitik retrospektif untuk menilai akurasi ADC maps intratumoral pada pemeriksaan MRI kepala sekuens ADC yang merupakan parameter kuantitatif sekuens DWI dalam menentukan derajat astrositoma. Nilai ADC diambil dengan menempatkan Region of Interest (ROI) dengan teknik free hand pada sekuens ADC mencakup area intratumoral tanpa mengenai area kistik, perdarahan dan nekrotik. Akurasi diagnostik nilai ADC maksimum, minimum dan mean ADC intratumoral untuk menentukan derajat astrositoma menggunakan analisis Receiver Operating Characteristic (ROC). Hasil. Didapatkan 27 pasien astrositoma derajat tinggi dan 9 pasien astrositoma derajat rendah. Berdasarkan analisis ROC, pengukuran area intratumoral dari nilai mean ADC intratumoral menghasilkan cutoff value 1,04 x 10-3 mm2/s dengan sensitivitas 88,9%, dan spesifisitas 88,9%, akurasi 80%, nilai ADC maksimum menghasilkan cutoff value 1,27 x 10-3 mm2/s, dengan sensitivitas 85,2%, spesifisitas 88,9%, akurasi 77,5%, sedangkan nilai ADC minimum menghasilkan cutoff value 0,80 x 10-3 mm2/s, dengan sensitifitas 85,2%, spesifisitas 88,9%, akurasi 77,5%, untuk membedakan astrositoma derajat tinggi dan derajat rendah. Kesimpulan. Pengukuran nilai mean ADC intratumoral pada astrositoma menghasilkan nilai akurasi diagnostik tinggi dalam menentukan derajat keganasan astrositoma

Background: Astrocytoma is a primary brain tumor originating from astrocytes. Astrocytoma causes > 80% of brain tumors with the second high mortality rate so that the diagnosis can be made as early as possible. Astrocytoma consists of three parts, namely the entire tumor, solid tumor and peritumoral edema. Measurement of the accuracy of the diffusion value of the pparent Diffusion Coefficient (ADC) on intratumoral mapping on MRI imaging can determine the degree of malignancy of astrocytoma. This examination is non-invasive compared to histopathological examination in determining the degree of astrocytoma. Purpose: The purpose of this study was to determine the accuracy of the diffusion value of intratumoral ADC maps on contrast head MRI imaging compared to the gold standard of Anatomical Pathology in determining the degree of astrocytoma. Method: A retrospective analytic observational study to assess the accuracy of intratumoral ADC maps on head MRI examination of ADC which is a quantitative parameter of DWI in determining the degree of astrocytoma. The ADC value is taken by placing the Region of Interest (ROI) with a free hand technique on the ADC sequence covering the intratumoral area without affecting the cystic, bleeding and necrotic areas. Diagnostic accuracy of maximum, minimum and mean intratumoral ADC values to determine the degree of astrocytoma using Receiver Operating Characteristic (ROC) analysis. Results. There were 27 high-grade astrocytoma patients and 9 low-grade astrocytoma patients. Based on the ROC analysis, measurement of the intratumoral area of the mean intratumoral ADC resulted in a cutoff value of 1,04 x 10-3 mm2/s with sensitivity of 88,9% and specificity of 88,9%, accuracy 80%, maximum ADC value resulted in a cutoff value of 1,27 x 10-3 mm2/s, with a sensitivity 85,2%, specificity of 88,9% accuracy 77,5%, while the minimum ADC value has a cutoff value of 0,80 x 10-3 mm2/s, with a sensitivity of 85,2%, and a specificity of 88,9% accuracy 77,5% to distinguish high and low grade astrocytomas Conclusion. Measuring the mean value of intratumoral ADC in astrocytoma results in a high diagnostic accuracy value in determining the degree of malignancy of astrocytomas.
