Faktor Human Error Penyebab Keterlambatan Penyediaan Rekam Medis Rawat Jalan di Rumah Sakit

Latar Belakang: Salah satu indikator mutu pelayanan rekam medis adalah waktu penyediaan dokumen rekam medis pasien rawat jalan kurang dari sama dengan 10 menit. Keterlambatan penyediaan penyediaan rekam medis mengakibatkan keterlambatan pemenuhan hak-hak pasien serta tidak sesuai dengan Standar Pelayanan Minimal Rumah Sakit. Tujuan: Menganalisis faktor penyebab keterlambatan penyediaan rekam medis di rumah sakit dari sisi human error pada penelitian yang sudah pernah dilakukan sebelumnya Metode: Jenis penelitian dengan metode studi literatur. Pencarian literatur dilakukan menggunakan Google Scholar dengan kata kunci yaitu keterlambatan, waktu, penyediaan, rekam medis, rawat jalan, rumah sakit, kesalahan manusia. Literatur diseleksi berdasarkan kriteria inklusi dan eksklusi. Hasil akhir digunakan 15 penelitian untuk di-review. Hasil: Berdasarkan hasil studi terhadap 15 penelitian yang berkaitan dengan faktor penyebab keterlambatan penyediaan rekam medis rawat jalan di rumah sakit, dari 36 faktor penyebab keterlambatan rekam medis rawat jalan di rumah sakit, 14 diantaranya termasuk ke dalam human error, 6 berupa violation dan 9 penyebab non-human error. Kesimpulan: Keterlambatan penyediaan rekam medis rawat jalan di rumah sakit disebabkan oleh human error sebagai faktor penyebab terbesar. Melakukan analisa lebih mendalam terhadap faktor human error sehingga langkah untuk mengatasi permasalahan keterlambatan penyediaan rekam medis di rumah sakit lebih tepat dan efisien. Background: One indicator of the service quality of medical record is the time for providing outpatient medical record documents less than equal to 10 minutes. The delay in providing of medical records resulted in delays in the fulfillment of patient rights and did not comply with the Hospital Minimum Service Standards. Objective: To analyze the factors that cause delays in providing medical records in hospitals from the side of human error in research that has been done before Methods: This type of research uses the literature study method. Literature search was conducted using Google Scholar as a database with keywords, namely delay, time, provision, medical records, outpatient, hospital, human error. The literature was selected based on inclusion and exclusion criteria. The final results used 15 studies as the literature reviewed. Results: Based on the results of a study of 15 studies related to factors causing delays in providing outpatient medical records in hospitals, of the 36 factors causing delays in outpatient medical records in hospitals, 14 of them were included in human error, 6 were violations and 9 causes non-human error. Conclusion: The delay in providing outpatient medical records at the hospital was caused by human error as the biggest factor. Conduct a more in-depth analysis of the human error factor so that steps to overcome the problem of delays in providing medical records in hospitals are more precise and efficient.
