LUARAN ANAK DENGAN NEFRITIS LUPUS ERITEMATOSUS, PENYAKIT GINJAL KRONIK STADIUM II, DISERTAI FLARE DIPICU DEMAM DENGUE

Latar Belakang Lupus Eritematosus Sistemik (LES) merupakan penyakit autoimun multisistem kronis dengan beragam manifestasi klinis, perjalanan penyakit, flare dan prognosis. Laki-laki dengan LES memiliki kemungkinan lebih besar untuk terkena gangguan renal. Pada daerah endemis, infeksi virus dengue diduga sebagai faktor yang dapat memicu flare pada LES. Tujuan Mengamati, memonitor, dan memberikan intervensi jangka panjang kepada pasien LES secara terpadu dan holistik untuk mencegah komplikasi, sehingga pasien mempunyai kualitas hidup dan prognosis yang lebih baik Metode Dilakukan pemantauan selama total 36 bulan, yaitu 18 bulan periode pengamatan pada pasien LES terhitung dari Desember 2015 sampai dengan Mei 2017 dan 18 bulan pasca periode pengamatan dari Mei 2017 sampai dengan Desember 2018. Hasil. Pasien menunjukkan remisi komplit yang dicapai setelah pasien menjalani protokol. Aktivitas penyakit dengan skor Systemic Lupus Erythematosus Disease Activity Index (SLEDAI) = 0 diluar periode flare. Fungsi ginjal membaik hingga LFG >= 90 ml/menit/1,73m2 . Kerusakan organ minimal dengan Systemic Lupus International Collaborating Clinics Disease Index (SDI) = 1 pada akhir pemantauan. Pertumbuhan pasien membaik, pada awal pengamatan sempat terjadi obesitas dan stunting, namun selanjutnya normal. Kualitas hidup meningkat, baik persepsi anak maupun orangtua pada aspek fisik, emosi, sosial dan sekolah. Selama periode pengamatan, terjadi dua kali flare yang dipicu infeksi virus dengue. Kesimpulan. Untuk mencapai luaran yang optimal terutama luaran fungsi renalpada pasien LES, dibutuhkan ke rjasama yang erat antara pasien, keluarga, tim medis, sekolah dan lingkungan sekitar. Diperlukan kewaspadaan yang tinggi terhadap flare yang dipicu dengue pada pasien LES yang tinggal di daerah endemis.

Background Systemic Lupus Erythematosus (SLE) is a multisystem autoimmune disease with broad range of clinical manifestation, course of disease, flare and prognosis. Male affected by SLE had higher risk to suffer from renal involvement. In endemic region, dengue viral infection was suggested as triggering factor that induce flare in SLE patient. Objective To observe, monitor and give to SLE patient in a holistic and integrated care to prevent complication so that patients quality of life and prognosis can be improved. Method Observation and management were performed for a total of 36 hours, consisted of 18 months of observation period from December of 2015 until May 2017 and 18 months of post observation period from May 2017 until December of 2018. Result Patient showed complete remission, achieved during protocol of SLE. At the end of observation period, disease activity reduced with Systemic Lupus Erythematosus Disease Activity Index = 0, outside of the flare period. Renal function improved with GFR >= 90 ml/minutes/1,73m 2 . Organ damage can be kept to minimum with Systemic Lupus International Collaborating Clinics Disease Index = 1. Growth was normal, with obesity and stunting that present at the start of observation eventually resolved. Quality of life was improved by both child and parents perception at physical, emotional, social and school aspect. During observation period, there were two episodes of flare induced by dengue infection. Conclusion in order to acquire optimal outcome, especially renal outcome of SLE patient, collaboration between patient, family, medical team, school and neighborhood were needed. Utmost vigilante must be taken for dengue induced flare for SLE patient that live in endemic area.
