PENGARUH PLATELET RICH FIBRIN KOMPRESI DENGAN DAN TANPA RELEASATE PADA BEDAH MUKOGINGIVAL PERAWATAN RESESI GINGIVA Kajian Klinik

Perawatan resesi gingiva adalah penutupan akar gigi yang terbuka dan memperbaiki masalah estetika. Mempertahankan level penutupan akar dengan regenerasi jaringan merupakan elemen penting dalam bedah plastik periodontal. Platelet rich fibrin (PRF) mengandung banyak faktor pertumbuhan yang berperan dalam regenerasi jaringan. PRF biasanya digunakan dalam bentuk kompresi. Hasil samping PRF kompresi adalah releasate yang kaya akan faktor pertumbuhan. Tujuan dari penelitian ini adalah untuk mengetahui pengaruh PRF kompresi dengan dan tanpa releasate pada bedah mukogingival perawatan resesi gingiva. Pada penelitian ini sampel dibagi menjadi dua kelompok yaitu kelompok kontrol yaitu coronaly advanced flap (CAF) dengan PRF kompresi dan kelompok perlakuan yaitu CAF dengan PRF kompresi dikombinasikan dengan releasate. Pengukuran dilakukan pada parameter klinis recession depth, probing depth, relative attachment level, keratinized tissue width, dan gingival thickness yang diukur pada baseline, minggu keempat, dan minggu kedua belas. Hasil uji Friedmann dan Mann Whitney menunjukkan perbedaan yang bermakna pada parameter recession depth, relative attachment level, keratinized tissue width, dan gingival thickness antar waktu pengukuran. Hasil uji Kruskal wallis dan Mann Whitney menunjukkan adanya perbedaan yang bermakna pada parameter recession depth, relative attachment level, dan gingival thickness antar kelompok. Tidak ditemukan perbedaan yang bermakna pada parameter probing depth dan keratinized tissue width. Kesimpulan yang diperoleh dari penelitian ini penambahan releasate pada PRF dapat meningkatkan keberhasilan perawatan resesi gingiva menggunakan bedah mukogingival terhadap parameter recession depth, relative attachment loss, dan gingival thickness, namun tidak berpengaruh terhadap probing depth dan keratinized tissue width.
Gingival recession is the apical migration of the gingival margin from cementoenamel junction. The treatment of gingival recession is to cover tooth root that is exposed and fix the aesthetics problem. Maintaining the coverage level of the root with tissue regeneration is an important element in periodontal plastic surgery. Platelet rich fibrin (PRF) contains many growth factors which trigger tissue regeneration. PRF is usually used in compressed form. Byproducts compression PRF is releasate which is rich in growth factors. The purpose of this study was to determine the effect of compressed PRF with and without releasate on treatment of gingival recession by mucogingival surgery. In this study, the sample was divided into two groups: the control group coronaly advanced flap (CAF) with compressed PRF and treatment group CAF with compressed PRF combined with releasate. Measurements were made on clinical parameters include recession depth, probing depth, relative attachment level, keratinized tissue width, and gingival thickness which were measured at baseline, four weeks, and the twelfth week. The results of Friedmann and Mann Whitney test showed significant differences in recession depth, relative attachment level, keratinized tissue width, and gingival thickness between the measurement time. Results Kruskal Wallis and Mann Whitney test showed significant differences in recession depth, relative attachment level and gingival thickness between groups. There was no significant differences in probing depth and keratinized tissue width. The conclusions of this study were the addition releasate on PRF can improve the success of mucogingival surgical treatment of gingival recession using the parameter recession depth, relative attachment loss and gingival thickness, but has no effect on probing depth and keratinized tissue width.
