KOMPLIKASI PADA BAYI BARU LAHIR DAN KONSEKUENSINYA TERHADAP BIAYA PERAWATAN NEONATAL JKN RAWAT INAP DI RSUP Dr. SARDJITO YOGYAKARTA

Pasien neonatal, berisiko mengalami komplikasi yang memerlukan biaya perawatan tinggi. Pada era Jaminan Kesehatan Nasional (JKN), pembiayaan kesehatan pada Fasilitas Kesehatan Rujukan Tingkat Lanjutan (FKRTL) diberlakukan tarif INA-CBGs. Tetapi seringkali tarif rumah sakit lebih besar dari tarif INA-CBGs sehingga dapat menyebabkan kerugian pada pihak rumah sakit. Penelitian ini bertujuan untuk mengetahui komplikasi, biaya perawatan, perbedaan antara tarif rumah sakit dengan tarif INA-CBGs, dan faktor yang mempengaruhi tarif rumah sakit pasien neonatal JKN rawat inap di RSUP Dr. Sardjito Yogyakarta dari perspektif rumah sakit. Penelitian ini merupakan penelitian analitik. Subyek penelitian adalah seluruh pasien neonatal JKN rawat inap Bulan Januari-Juni 2015. Data berasal dari berkas klaim dan rekam medik pasien yang diambil secara retrospektif. Dilakukan pencatatan yang berisi karakteristik pasien, diagnosis, tindakan/prosedur, dan rincian biaya perawatan pasien. Analisis komplikasi yang terjadi dilakukan menggunakan statistik deskriptif, perbedaan tarif INA-CBGs dengan tarif rumah sakit menggunakan one sample t test, dan faktor yang mempengaruhi besarnya tarif rumah sakit menggunakan korelasi Spearman. Hasil penelitian mencatat total 307 pasien neonatal dengan 309 periode rawat inap. Ditemukan 246 macam diagnosis, 85 macam tindakan, dan 86 kelompok INA-CBGs beserta kelas perawatannya. Komplikasi yang sering terjadi antara lain neonatal jaundice, bacterial sepsis, hipoglikemia dan respiratory distress syndrome. Biaya perawatan pasien memiliki rentang tarif Rp 472.000,00 sampai dengan Rp 117.791.574,98. Total tarif rumah sakit seluruh pasien Rp 4.345.265.383,69 dengan biaya pemeriksaan dokter, konsulen dan visite sebagai komponen biaya terbesar yang berkontribusi sebesar 27,25% dari total biaya. Selisih tarif rumah sakit dengan tarif paket INA-CBGs sebesar Rp 1.191.392.983,69 atau 27,4% dari total biaya. Faktor-faktor yang mempengaruhi tarif rumah sakit adalah usia kehamilan, berat badan lahir, jumlah diagnosis, jumlah tindakan/prosedur, dan lama perawatan di rumah sakit. Semakin banyak jumlah komplikasi yang dialami pasien, semakin besar tarif rumah sakit pasien.

Neonatal patients are at risk of developing complications that require high care costs. In the era of National Health Insurance (JKN), health financing at Advanced Level Referral Health Facility (FKRTL) is based on INA-CBGs rates. But often the hospital rates greater than INA-CBGs rates. It causes financial risk to the hospital. This study aims to determine the complications, neonatal care cost, the difference between hospital rates and INA-CBGs rates, and factors that affect the hospital rates of JKN neonatal patients hospitalized in Dr. Sardjito Hospital from the perspective of the hospital. This research is analytic. The subjects were all JKN neonatal patients hospitalization in January to June 2015. The data comes from the claim file and medical records of the patients were obtained retrospectively. It was recorded patient characteristics, diagnosis, actions/procedures, and details of patient care cost. Analysis of complications are conducted using descriptive analysis, differences in rates between INA-CBGs rates and hospital rates was analized using one sample t test, and factors that influence the hospital tariff was analized using Spearman correlation. The results recorded a total of 307 patients with 309 neonatal hospitalization period. Found 246 kinds of diagnosis, 85 kinds of action, and 86 groups of INA-CBGs with its class treatment. A frequent complications include neonatal jaundice, bacterial sepsis, hypoglycemia and respiratory distress syndrome. The cost of patient care ranges IDR 472,000.00 to IDR 117,791,574.98. Total hospital tariff was IDR 4,345,265,383.69 with a doctor's examination, consultant and visite fees as the largest cost component which contributed 27.25% of the total cost. Difference in rates between hospital and INA-CBGs rate was IDR 1,191,392,983.69 or 27.4% of the total cost. Factors that affect the hospital tariff are gestational age, birth weight, the number of diagnoses, the number of actions/procedures, and length of stay. The more the number of complications experienced by patients, the greater the patient's hospital rate.
