PELAKSANAAN KREDENSIAL PEREKAM MEDIS DI RSUD Dr. SOEDIRMAN KABUPATEN KEBUMEN MENGACU PADA STANDAR KPS 15 AKREDITASI 2012

Latar belakang: Menurut Huffman (1994), anggota tenaga kesehatan merupakan subjek tenaga kesehatan berdasarkan hukum, peraturan, dan perundang-undangan serta program merupakan program untuk meningkatkan kualitas rumah sakit. Oleh karena itu, aktivitas tenaga kesehatan seharusnya dikoordinasi dengan kredensial. Selain itu, pada Standar KPS 15 Akreditasi 2012 menyatakan bahwa rumah sakit mempunyai standar prosedur untuk mengumpulkan, memverifikasi, dan mengevaluasi kredensial staf kesehatan profesional lainnya (izin, pendidikan, pelatihan, dan pengalaman). Sesuai standar tersebut, Perekam Medis termasuk ke dalam jenis staf kesehatan profesional lainnya, sehingga untuk mencapai elemen penilaian dalam Standar KPS Akreditasi 2012, Perekam Medis harus dilakukan kredensial. Tujuan Penelitian: Tujuan secara umum dalam penelitian ini adalah mengetahui pelaksanaan kredensial Perekam Medis di RSUD Dr. Soedirman Kabupaten Kebumen. Tujuan khususnya adalah mengetahui uraian tugas Perekam Medis, mengetahui dokumen terkait kredensial Perekam Medis, dan mengetahui pelaksanaan proses kredensial Perekam Medis di RSUD Dr. Soedirman Kabupaten Kebumen. Metode Penelitian: Jenis penelitian yang dilakukan adalah penelitian deskriptif dengan pendekatan kualitatif. Rancangan penelitian menggunakan studi kasus. Teknik pengambilan data yang digunakan adalah studi dokumentasi, wawancara, dan observasi. Kemudian, untuk teknik validasi data, peneliti menggunakan triangulasi sumber. Hasil: Proses kredensial dilakukan untuk menguji kembali apakah Perekam Medis tersebut telah mampu melakukan uraian tugas sesuai kompetensi tanpa pendampingan. Uraian tugas Perekam Medis di RSUD Dr. Soedirman Kabupaten Kebumen tertulis dalam formulis sasaran kerja pegawai (SKP). Kemudian, untuk dokumen terkait kredensial Perekam Medis terdiri dari dokumen regulasi dan dokumen proses kredensial. Dokumen regulasi yang dimiliki RSUD Dr. Soedirman Kabupaten Kebumen terkait kredensial Perekam Medis adalah kebijakan, pedoman/panduan, dan SPO. Kemudian, untuk dokumen proses kredensial berupa dokumen proses praasessmen, dokumen proses asessmen kompetensi, dan dokumen penerbitan surat kewenangan klinis. Pelaksanaan proses kredensial Perekam Medis di RSUD Dr. Soedirman Kabupaten Kebumen terdiri dari proses praasessmen, proses asessmen kompetensi, dan proses penerbitan surat kewenangan klinis. Pelaksanaan kredensial tersebut dilakukan oleh Perekam Medis yang menjadi Tim Kredensial Tenaga Kesehatan Lain. Kesimpulan: Uraian tugas Perekam Medis di RSUD Dr. Soedirman Kabupaten Kebumen tertulis daIam formulir sasaran kerja pegawai (SKP). Dokumen terkait kredensial Perekam Medis terdiri dari dokumen regulasi dan dokumen proses kredensial. Pelaksanaan proses kredensial terdiri dari proses praasessmen, proses asessmen kompetensi, dan proses penerbitan surat kewenangan klinis.

Background: According to Huffman (1994), member of medical staffs is the subject of medical staff based on law, rules, regulation, and program is the agenda to increase the quality of hospital. Therefore, the activity of medical staff should be credentially coordinated. Besides, in the Standar KPS Akreditasi 2012 mentioned that hospital has the standard of procedure to gather, verivicate, and evaluate medical staff’s credentially (permit, education, training, and experience). Based on that standard, Medical Recorder is included into the other profesional medical staff, which consequently to achieve asessment element in Standar KPS Akreditasi 2012 Medical Recorder is required to be credential. Research Objective: The general objective of this research is to know the implementation of the credentials of Medical Recorder at RSUD Dr. Soedirman Kabupaten Kebumen. The specific objectives of this research, then, are to explain the job description of Medical Recorder in RSUD Dr. Soedirman Kabupaten Kebumen, and to explain the related document regarding to credential of Medical Recorder in RSUD Dr. Soedirman Kabupaten Kebumen, and to explain the conduct of credentialing process of Medical Recorder in RSUD Dr. Soedirman Kabupaten Kebumen. Research Method: The research is descriptive type research which uses qualitative approach. The research framework is using case study. The data collection techniques are through documentation study, interview, and observation. Furthermore, for data validating technique the writer use source triangulation. This research was done in RSUD Dr. Soedirman Kabupaten Kebumen. Result: The credentials process do to retest whether the Medical Recorder has been able to do the job description according to the competence without accompaniment. The job description of Medical Recorder at RSUD Dr. Soedirman Kabupaten Kebumen written in the formulis of the work targets employee (SKP). Then, for documents that related to the credentials of Medical Recorder consists of regulatory documents and document processes of credentials. The regulatory documents that owned by RSUD Dr. Soedirman Kabupaten Kebumen that related to the credentials of Medical Recorder is a policy, guidances/guides, and SPO. Then, to the documents processes of credential formed by praasessmen process documents, documents of competence asessmen process, and document issuance clinical authority. Implementation of the credentials of Medical Recorder in RSUD Dr. Soedirman Kabupaten Kebumen consists of praasessmen process, competence asessmen process, and the process of issuance of the clinical authority. Implementation of these credentials is done by the Medical Recorder which became Credentials Team of the Other Health Workers. Conclusion: The job description of Medical Recorder in RSUD Dr. Soedirman Kabupaten Kebumen was written on form of work targets employe. Documents that related to credentials of Medical Recorder consists of regulatory documents and document processes credentials. Implementation of the credentials process consists of praasessmen process, competence asessmen process, and the process of issuance of the clinical authority.
