KETEPATAN KODE DIAGNOSIS PENYEBAB DASAR KEMATIAN BERDASARKAN ICD-10 DI RSUP DR. SOERADJI TIRTONEGORO KLATEN

Latar Belakang: Aturan untuk penentuan diagnosis penyebab dasar kematian menurut WHO adalah berdasarkan ketentuan yang ada dalam ICD-10 dilengkapi dengan Tabel MMDS sebagai alat crosscheck hubungan sebab-akibat (kausal). Berdasarkan studi pendahuluan, masih ditemukan kode "R" dalam laporan 10 besar sebab kematian. Hal ini berarti kode diagnosis penyebab dasar kematian masih belum sesuai aturan pemilihan penyebab dasar kematian berdasarkan ICD-10. Tujuan: Mengetahui pelaksanaan pengodean diagnosis penyebab dasar kematian di RSUP Dr. Soeradji Tirtonegoro Klaten, mengetahui persentase ketepatan kode diagnosis penyebab dasar kematian berdasarkan ICD-10, mengetahui faktor-faktor ketidaktepatan, dan mengetahui upaya yang telah dilakukan untuk memperbaiki alur penentuan kode diagnosis penyebab dasar kematian di RSUP Dr. Soeradji Tirtonegoro Klaten. Metode Penelitian: Jenis penelitian yang digunakan adalah penelitian deskriptif dengan pendekatan kualitatif dan rancangan penelitian adalah studi kasus. Subjek pada penelitian ini adalah dua orang coder, satu orang dokter rawat inap, dan satu orang dokter jaga IGD. Objek dalam penelitian ini yaitu SOP, Surat Keterangan Sebab Kematian, Resume, dan Laporan Kematian Tahun 2015. Hasil: Diagnosis penyebab dasar kematian di RSUP Dr. Soeradji Tirtonegoro Klaten ditentukan oleh dokter kemudian dikode oleh coder dengan menggunakan ICD-10 tanpa melakukan reseleksi sesuai aturan atau rule penentuan penyebab dasar kematian. Dari 79 data kematian, 44,30% kode diagnosis penyebab dasar kematian sudah tepat, dan 55,70% tidak tepat. Ketidaktepatan tersebut karena faktor man, machine, dan method. Upaya yang telah dilakukan untuk memperbaiki alur penentuan kode adalah koordinasi coder dengan dokter. Kesimpulan: Pelaksanaan penentuan kode diagnosis penyebab dasar kematian belum sesuai dengan prosedur dalam ICD-10. Persentase ketidaktepatan kode juga masih tinggi. Faktor ketidaktepatan yaitu man, machine, dan method. Untuk memperbaiki alur penentuan kode telah dilakukan upaya koordinasi antara coder dengan dokter.

Background: The rules for determining the diagnosis of the underlying causes of death according to the WHO is based on the provisions of the ICD-10 is equipped with MMDS table as a means to crosscheck the causal relationship (causal). Methods: This study is a descriptive study with qualitative approach and the study design was a case study. Subjects in this study were two coder, one doctor hospitalization, and one emergency call doctor. Objects in this research that SOP, Certificate For Death, Resume, and Death Reports 2015. Results: The diagnosis of the underlying causes of death in RSUP Dr. Soeradji Tirtonegoro Klaten determined by a doctor and then coded by coder using ICD- 10 without choose according to the rule or the rule of determining the underlying causes of death.
