Latar Belakang: Akreditasi puskesmas adalah suatu penilaian dan pengakuan yang diberikan oleh lembaga pemerintah untuk menjamin mutu pelayanan puskesmas. Isi rekam medis dan penilaian kelengkapan dan ketepatan isi rekam medis merupakan bagian dari aspek yang dinilai dalam standar akreditasi. Berdasarkan hasil wawancara dengan kepala puskesmas, diperoleh informasi bahwa Puskesmas Playen I sedang melakukan persiapan untuk akreditasi tahun 2017. Berdasarkan kegiatan pengabdian dibagian pendaftaran rawat jalan, diketahui bahwa banyak ditemukan data yang tercatat pada rekam medis rawat jalan yang telah selesai digunakan dalam pelayanan belum terisi dengan lengkap. Namun, tidak terdapat petugas yang melakukan penilaian kelengkapan dan ketepatan isi rekam medis. Tujuan: Penelitian ini bertujuan untuk mengetahui persiapan akreditasi dalam memenuhi elemen penilaian penilaian (EP)1 mengenai isi rekam medis dan elemen penilaian (EP) 2 mengenai penilaian dan tindak lanjut kelengkapan dan ketepatan isi rekam medis pada kriteria 8.4.4. di bagian rawat jalan Puskesmas Playen I Gunung Kidul. Metode Penelitian: Jenis penelitian yang digunakan adalah penelitian deskriptif dengan pendekatan kualitatif dan dengan rancangan penelitian fenomenologis. Subjek penelitian adalah, koordinator pendaftaran rawat jalan, petugas rawat jalan, ketua pokja pelayanan medik dan bendahara tim akreditasi. Teknik pengumpulan data dalam penelitian ini menggunakan teknik wawancara, observasi dan studi dokumentasi. Untuk validasi data menggunakan triangulasi sumber. Hasil: Penelitian ini menunjukkan persiapan Puskesmas Playen I untuk memenuhi kriteria 8.4.4 pada elemen penilaian pertama dan kedua. Pada elemen penilaian pertama telah terdapat persiapan yang dilakukan yaitu dari segi kebijakan, telah dilakukan pembagian tugas penyusunan kebijakan, dan penyusunan format baku penulisan surat keputusan (SK). Pada segi sumber daya manusia, telah terdapat sosialisasi mengenai cara pengisian rekam medis pada seluruh petugas. Pada segi peralatan, telah terdapat penyusunan draft formulir rawat jalan baru dengan item data yang lebih mengacu pada standar akeditasi. Pada segi anggaran biaya, telah terdapat penyusunan anggaran biaya untuk pencetakan formulir rawat jalan. Pada segi tahapan akreditasi, telah terdapat pembentukan tim akreditasi puskesmas, namun belum ada penyusunan daftar rencana kegiatan. Persiapan untuk memenuhi elemen penilaian kedua yaitu pada segi kebijakan telah terdapat penyusunan standar prosedur operasional (SPO) penilaian kelengkapan dan ketepatan isi rekam medis. Pada segi sumber daya manusia, telah terdapat rencana untuk mengikutsertakan petugas dalam pelatihan. Namun, dari segi peralatan, belum terdapat penyusunan formulir lembar kekurangan. Pada segi anggaran biaya, teah terdapat penyusunan anggaran untuk pembiayaan pelatihan bagi petugas. Pada segi tahap persiapan telah terdapat pembentukan tim akreditasi puskesmas, namun belum ada penyusunan daftar rencana kegiatan. Kesimpulan : Persiapan pada elemen penilaian pertama dari segi sumber daya manusia telah terpenuhi, persiapan dari segi peralatan telah terpenuhi, dan persiapan dari anggaran biaya telah terpenuhi, namun persiapan dari kebijakan dan persiapan dari segi tahap persiapan belum terpenuhi dengan baik. Untuk memenuhi elemen penilaian kedua, persiapan dari segi kebijakan telah terpenuhi, persiapan dari segi sumber daya manusia telah terpenuhi, dan persiapan dari segi anggaran telah terpenuhi. Namun persiapan dari segi peralatan dan tahapan persiapan belum terpenuhi dengan baik.

Background: Accreditation is an assessment of puskesmas and the recognition given by government agencies to ensure the quality of puskesmas services. The contents of the medical records and assessment of the completeness and accuracy of medical records is part of the aspects that the accreditation standards. In interviews with the heads of puskesmas, Puskesmas Playen I obtained information that this puskesmas was preparing for accreditation in 2017. Based on service activities section outpatient registration, it is known that many found the data recorded on the outpatient medical record that has been completed is placed in service has not been filled with complete. However, there are officers who assess the completeness and accuracy of medical records. Objective: This study aimed to determine the accreditation preparation in accomplishing the elements of assessment ratings (EP) 1 regarding the contents of the medical records and assessment elements (EP) 2 on the assessment and follow-up completeness and accuracy of medical records on criteria 8.4.4. at the outpatient health centers Playen I Gunung Kidul. Method: This study is a descriptive study with qualitative approach and phenomenological research design. Subjects were, coordinator of outpatient registration, outpatient care attendant, medical service working group chairman and treasurer accreditation team. Data collection techniques in this study using the technique of interview, observation and documentation. To validate data using triangulation. Results: This study demonstrates the preparation of Puskesmas Playen I to accomplish the criteria of 8.4.4 on the elements of the first and second votes. In the first assessment element has been found that the preparations made in terms of policy, there has been the division of labor policy formulation and preparation of raw formats writing of the decree (SK). In terms of human resources, there has been a socialization about how to fill in the whole medical records clerk. In terms of equipment, there have been drafting new outpatient form with the data item that refers more akeditasi standards. In terms of budget, there have been drafting budgets for outpatient forms printing. In terms of the stages of accreditation, there have accreditation team building health centers, but there is no preparation of the action plan. Preparations to meet the elements of the second assessment, namely in terms of policy there has been a standard preparation of operating procedures (SOP) assessment of the completeness and accuracy of medical records. In terms of human resources, there has been a plan to include officers in training. However, in terms of equipment, there has been no shortage of preparation sheet form. In terms of budget, there are budgeting for financing training for officers. In terms of the preparation phase has been contained accreditation team building health centers, but there is no preparation of the action plan. Conclusion: Preparation of the first assessment element in terms of human resources has been met, the preparation of the terms of the equipment are met, and the preparation of the budget have been met, but the preparation of policies and preparation of the terms of the preparation phase has not been fulfilled properly. To meet the elements of the second assessment, preparation of the terms of the policy are met, preparations in terms of human resources has been fulfilled, and the preparation of the terms of the budget has been met. However, preparations in terms of equipment and preparation stage have not met properly.
