PELAKSANAAN PENGISIAN LEMBAR RESUME MEDIS PASIEN RAWAT INAP DI RUMAH SAKIT ISLAM PKU MUHAMMADIYAH KABUPATEN TEGAL

Latar belakang: Kelengkapan pengisian dokumen merupakan hal yang sangat penting karena berhubungan dengan kesinambungan data dan mempengaruhi kualitas dalam pelayanan suatu rumah sakit. Tetapi Rumah Sakit Islam PKU Muhammadiyah Kabupaten Tegal, banyak terjadi ketidaklengkapan pengisian lembar resume medis. Tujuan: Tujuan penelitian ini adalah untuk mengetahui pelaksanaan alur pengisian lembar resume medis pasien rawat inap, mengetahui persentase kelengkapan pengisian lembar resume medis pasien rawat inap, dan mengetahui faktor apa saja yang menyebabkan ketidaklengkapan pengisian lembar resume di Rumah Sakit Islam PKU Muhammadiyah Kabupaten Tegal. Metode: Penelitian ini menggunakan metode Deskriptif dengan pendekatan Kualitatif dan rancangan penelitian Studi Kasus. Pengambilan data melalui Observasi, Studi Dokumentasi dan Wawancara . Hasil: Pelaksanaan alur pengisian lembar resume medis dilakukan setelah pasien diizinkan pulang oleh dokter DPJP. Tetapi di Rumah Sakit Islam PKU Muhammdiyah Kabupaten Tegal, perawat bangsal juga ikut mengisi lembar resume medis pasien rawat inap. Persentase kelengkapan pengisian lembar resume medis adalah 0% untuk kategori lengkap, 92,73% atau setara dengan 217 lembar resume medis untuk kategori tidak lengkap, dan 7,26% atau setara dengan 17 lembar resume medis untuk kategori tidak ada. Faktor-faktor penyebab ketidaklengkapan pengisian lembar resume medis antara lain karena kesibukan dokter, kurangnya kedisiplinan dokter, kurangnya kesadaran dokter terkait pentingnya kelengkapan pengisian lembar resume medis, tidak adanya SOP, serta tidak adanya penghargaan maupun sanksi bagi dokter yang mengisi lembar resume secara lengkap maupun yang tidak lengkap. Kesimpulan: Pelaksanaan alur pengisian lembar resume medis belum sesuai dengan peraturan yang ada. Hal ini juga berpengaruh terhadap kelengkapan pengisian lembar resume medis, sehingga persentasenya masih belum mencapai 100%. Hal ini disebabkan oleh kurangnya kedisiplinan dan kesadaran dokter terkait kelengkapan pengisian lembar resume medis. Selain itu, tidak adanya SOP serta tidak adanya sanksi tegas bagi dokter yang tidak mengisi lembar resume secara lengkap juga dapat menjadi faktor penyebab ketidaklengkapan pengisian lembar resume medis. Background: The completeness of the charging document is very important because it relates to sustainability and affect the quality of data in the service of a hospital. At the time he did a preliminary study at the Islamic Hospital PKU Muhammadiyah Kabupaten Tegal, a lot happened incompleteness charging discharge summary sheet. Objectives: The purpose of this research is to know the implementation of the charging sheet flow discharge summary inpatients, knowing the percentage of completeness of the charging sheet discharge summary inpatients, and find out what factors cause incomplete charging sheet resumes in the Islamic Hospital PKU Muhammadiyah Kabupaten Tegal. Methods: This research is a descriptive method using a qualitative approach to research and draft case studies. Data retrieval through Observation, Interview and Documentation Study. Results: The implementation of a charging sheet flow discharge summary done after the patient is discharged by the physician DPJP. But at the Islamic Hospital PKU Muhammdiyah Kabupaten Tegal, nurses ward also fill resume medical inpatients. The percentage of completeness of discharge summary sheet charging is 0% for the category of complete, 92.73% or equivalent to 217 discharge summary sheet for categories is not complete, and 7.26% or the equivalent of 17 discharge summary sheet for the category doesn't exist. Factors cause incomplete charging sheet discharge summary among other things because of the flurry of doctors, lack of discipline and lack of awareness of physicians, doctors linked the importance of completeness of the charging sheet discharge summary. the absence of procedures , and the absence of rewards or punishments for doctors who fill out a resume complete or incomplete . Conclusion: the implementation of a charging sheet flow discharge summary has not been in accordance with the existing regulations. This also affects the completeness of the charging sheet discharge summary, so the percentage is still haven't reached 100%. This is caused by a lack of discipline and related physician awareness of the completeness of the charging sheet discharge summary. Moreover , the absence of procedures and the absence of strict punishments for doctors who do not fill out a complete resume can also be a factor incompleteness medical resume charging sheet .
