RANCANGAN FORMULIR GIGI DAN MAP REKAM MEDIS DI BALAI KESEHATAN LEMBAGA PEMASYARAKATAN KELAS II A YOGYAKARTA

Latar Belakang: Salah satu fungsi penyimpanan berkas rekam medis bertujuan untuk mempermudah dan mempercepat ditemukan kembali berkas rekam medis yang disimpan dalam rak filing. Formulir gigi berfungsi sebagai daftar riwayat penyakit pasien. Balai Kesehatan Lembaga Pemasyarakatan kelas II A Yogyakarta hanya memiliki formulir rekam medis dan belum memiliki map / sampul rekam medis yang membuat formulir bisa tercecer, terlipat bahkan bisa hilang dan belum adanya formulir gigi untuk poli gigi. Tujuan: Menghasilkan desain map berkas rekam medis dan formulir gigi sesuai kebutuhan petugas pada bagian penyimpanan atau filling dan dokter gigi di Balai Kesehatan Lembaga Pemasyarakatan kelas II A Yogyakarta. Metode Perancangan: Perancang melakukan studi pendahuluan di Balai Kesehatan Lembaga Pemasyarakatan Kelas II A Yogyakarta dan menentukan akan melakukan desain ulang formulir Klinik Gigi dan desain map rekam medis. Kemudian dilakukan identifikasi formulir yang telah ada di Balai Kesehatan Lembaga Pemasyarakatan Kelas II A Yogyakarta dengan wawancara kepada responden. Setelah mengetahui hal yang perlu diperbaiki, perancang membuat Dua alternatif desain formulir Klinik gigi dan map rekam medis dan diajukan kepada responden untuk dipilih dan diberi masukan. Desain terpilih dilakukan revisi dengan memperhatikan masukan dari responden. Setelah revisi, perancang menunjukkan kembali hasil desain terpilih yang telah direvisi kepada responden untuk disetujui. Hasil: Item dalam formulir gigi di Balai Kesehatan Lembaga Pemasyarakatan Kelas II A Yogyakarta sesuai dengan teori Departemen Kesehatan RI (2004) hanya ada penambahan item yaitu pejabat yang menahan dan perkara / pasal. Serta mendesain map rekam medis dan desain terpilih untuk formulir gigi dan map rekam medis menggunakan ivory 250 gram dengan warna kuning, biru, dan orange. Ukuran kertas dapat dilipat sehingga membentuk sampul. Judul REKAM MEDIS menggunakan huruf TIMES NEW ROMAN BOLD DENGAN UKURAN 24 POINT. Lalu formulir gigi menggunakan kertas HVS 70 gram berwarna putih dengan ukuran A5 setiap formulir memiliki nomer halaman dan memiliki lubang fastener, batas tepi atas 1 cm, bawah 1 cm, kanan dan kiri 1 cm. Background : One of the medical record file storage is function to facilitate and accelerate rediscovered medical record file. that is stored in the rack gear filing. Form serves as a list of the patient's medical history . Health Center Penitentiary Class II A Yogyakarta only has a medical record form and didn't have a folder / cover medical record, that makes forms can be dislodged, folded, and even can be lost and there is no form. Purpose: to design file folder of medical records and dental forms as needed officers at the storage or filling and dentists in the Health Center Penitentiary Class II A Yogyakarta . Method : The designer did a preliminary study on Health Center Penitentiary Class II A Yogyakarta and decisive will to redesign the form and design of Dental Clinic medical records folder . Then to identify the form that has existed in the Health Center Penitentiary Class II A Yogyakarta with interviews with respondents . After knowing the things that need to be corrected , the designer create two alternative designs and folders form dental clinic medical records and asked the respondent to be selected and given input . Design will revised with input from respondents . After the revision , the designer showed again chosen design results revised to respondents for approval. Results : The items in the dental form in the Health Center Penitentiary Class II A Yogyakarta in accordance with the theory of the Department of Health (2004 ) is only the addition of items that officials who hold and case / article . As well as medical records folder design and the design chosen for the dental form and map using the medical records of 250 grams ivory yellow , blue , and orange . The size of the paper can be folded so as to form the cover . Title MEDICAL RECORD use TIMES NEW ROMAN BOLD letters SIZE WITH 24 POINTS .
