PENGARUH APLIKASI TOPIKAL PROPOLIS 10% TERHADAP PENYEMBUHAN LUKA PASCA PENCABUTAN GIGI DESIDUI PERSISTENSI (Kajian Klinis Pada Anak Usia 6-10 Tahun)

Pencabutan gigi berakibat luka yang memerlukan beberapa hari untuk penyembuhan. Proses penyembuhan luka diawali fase inflamasi, fase proliferasi dan fase maturasi. Pada fase proliferasi terjadi epitelisasi dan kontraksi luka. Aplikasi bahan yang bersifat antibakteri, antiinflamasi dan antioksidan dapat mempercepat penyembuhan luka. Propolis mengandung senyawa flavonoid dan fenol (Caffeic Acid Phenethyl Ester). Tujuan penelitian adalah mengetahui pengaruh aplikasi topikal propolis 10% terhadap penyembuhan luka pasca pencabutan gigi desidui persistensi. Penelitian eksperimental semu dilakukan pada 24 luka pasca pencabutan, 16 dengan aplikasi topikal propolis (perlakuan) dan 8 tanpa aplikasi (kontrol). Aplikasi dilakukan 2 kali sehari pada siang dan malam selama 3 hari. Tingkat penyembuhan luka diamati secara klinis dengan parameter luas luka relatif terhadap luka awal (%), degradasi warna luka dan simptom nyeri. Pengamatan dilakukan pada hari pertama, ketiga, kelima, ketujuh dan kesepuluh. Data didapatkan dari hasil foto digital luka pasca pencabutan gigi. Luas luka diukur dengan cara foto luka ditracing dan diukur dengan software Google SketchUp. Degradasi warna luka diukur dengan membandingkan jaringan sehat disekitarnya. Simptom nyeri diukur dengan ada atau tidaknya nyeri pasca pencabutan gigi. Data dianalisis dengan statistik deskriptif dan uji Mann-Whitney pada tingkat signifikansi 5%. Terdapat perbedaan bermakna (p<0,05) penurunan luas luka antara kelompok perlakuan dan kontrol pada hari ketiga sampai dengan kesepuluh. Degradasi warna luka pada kelompok perlakuan lebih cepat mendekati warna normal. Simptom nyeri tidak ditemukan pada kelompok perlakuan, sedangkan pada kelompok kontrol ditemukan sampai dengan hari ketiga. Disimpulkan bahwa aplikasi topikal propolis 10% dapat mempercepat penyembuhan luka pasca pencabutan gigi desidui persistensi.

Tooth extraction leave wound for several days to healing. This healing process of wound through inflammatory, proliferative and maturation phases. Propolis contains of flavonoids and phenols (Caffeic acid phenethyl ester) which contain antioxidant, antibacterial and antiinflammatory agent that may accelerate the wound healing process. The aim of the research was to know the effect of apply topically of 10% propolis on the wound healing of persistent deciduous extraction. The quasi-experimental research was conducted on the 24 post-extraction wound of persistent deciduous where the 16 cases were treat with topically application of 10% propolis twice a day at day and night time for three days post extraction, and the rest as control with no application. Healing of the wounds were clinically observed by measuring the wound wide with digital photo traced then analized with Google SkecthUp software; the color gradation of wound were compared with the normal tissue around; and the pain simptom based on the subject statement. The data were taken at the first, third, fifth, seventh and tenth days post tooth extraction. The data were statistically analized descriptively and Mann Whitney test at 5% signifficant level. There was a signifficant difference (p <0.05) of the wound width reduction between the treatment group and the control at the third to tenth days post tooth extraction; while the color gradation treatment group relative faster than the control and with no pain symptom. It was concluded that apply topically of 10% propolis can accelerate the wound healing post extraction of persistent deciduous tooth.
