ANALISIS BIAYA OBAT PADA ERA JAMINAN KESEHATAN NASIONAL DAN FAKTOR-FAKTOR YANG MEMPENGARUHI DI FASILITAS PENUNJANG KESEHATAN DAERAH ISTIMEWA YOGYAKARTA

Apotek merupakan salah satu fasilitas pelayanan penunjang kesehatan pada era Jaminan Kesehatan Nasional (JKN). Pada era JKN apotek dapat berperan sebagai apotek program rujuk balik (PRB) atau apotek jejaring dan apotek klinik pratama. Biaya obat apotek PRB diklaim langsung ke BPJS, sedangkan biaya obat apotek jejaring dan klinik pratama diklaim ke fasilias kesehatan primer berdasarkan hasil negoisasi. Variasi biaya obat pada era JKN sangat mempengaruhi profitabilitas apotek. Penelitian ini bertujuan untuk mengetahui rata-rata biaya obat, persentase biaya obat terhadap kapitasi, dan perbedaan biaya obat. Penelitian ini adalah penelitian observasi yang bersifat analitik cross sectional. Penelitian ini menggunakan data sekunder berupa resep pasien JKN untuk mengetahui rata-rata biaya obat, persentase biaya obat terhadap biaya kapitasi, dan perbedaan biaya obat. Penelitian ini dilakukan pada 6 apotek PRB, 6 apotek jejaring, dan 7 apotek klinik pratama di DIY pada bulan Oktober 2015. Subyek penelitian adalah resep bulan Maret 2015 dengan 8.430 lembar resep obat pasien JKN. Data penelitian biaya obat era JKN dianalisis secara statistik deskriptif dan uji komparatif (uji Kruskal Wallis). Hasil Penelitian menunjukkan bahwa, rata-rata biaya obat untuk satu kali terapi di apotek jejaring; apotek klinik pratama; apotek PRB masing-masing bernilai Rp 12.589±Rp 8.874,49; Rp 14.173 ± Rp 6.424,09; Rp 143.807 ± Rp 162.251,30. Rata-rata persentase biaya obat terhadap biaya kapitasi di apotek jejaring bernilai 13,58% dan apotek klinik pratama bernilai 15,91%. Terdapat perbedaan biaya obat yang signifikan di ketiga jenis apotek tersebut (p=0,000). Biaya obat di era JKN tergantung pada pola kerjasama apotek dengan dengan fasilitas kesehatan (faskes) dan BPJS. Rata-rata persentase biaya obat terhadap kapitasi faskes di apotek jejaring lebih rendah dibandingkan dengan apotek klinik pratama.

Pharmacy is one of supporting health care facility in the era of Jaminan Kesehatan Nasional (JKN). In JKN era, pharmacy can play roles in a form of affiliated pharmacies (apotek Program Rujuk Balik), or networking pharmacy (apotek jejaring) and in-house pharmacy (apotek klinik pratama). Affiliated pharmacy drug cost can be claimed directly to BPJS, mean while for the other type of pharmacy have to negotiation first with the primary health care. Drug cost variations in the JKN era affect the profitability of the business pharmacies. This research aims to determine the average cost of the drug, the drug percentage charges against capitation, and variety of drug costs. This research is analytic observational cross sectional. This research uses secondary data from a JKN prescription patient to determine the average cost of the drug, the variety in drug costs and the factors that influence the drug costs in JKN era. This research was conducted on 6 affiliated pharmacies, 6 networking pharmacies, and 7 in-house pharamcies in DIY. The subject of this research is 8.430 prescriptions, selected JKN prescriptions patient in March 2015. Data drug costs JKN era was analyzed by descriptive statistics and comparative test (Kruskal Wallis test). The result shows that on average the drug costs at the networking pharmacy, in-house pharmacies and insurer affiliated pharmacies each worth Rp12.589 ± Rp 8.874,49; Rp14.173 ± Rp 6.424,09; Rp143.807 ± Rp162.251,30. The average percentage of drug costs for capitation fee in the networking pharmacy is 13,58% and primary healthcare is 15,91%. Pharamcy in JKN era have drug cost variations (p = 0,000). Drug cost in JKN era depends on the pattern of play roles with the health facilities (faskes) and BPJS. The average percentage of drug costs against capitation faskes in networking pharmacy is lower than in-house pharmacy.
