Disability underlined to be the world concerned. All countries encouraged to mainstreaming disability issues especially in inclusive health services for persons with disabilities (PWD) in order to respect the human rights and to communicate with them effectively. In Indonesia, among PWD, Person with blindness (PWB) had high magnitude of concern compared to another disability classification. Improving effective communication, barriers should be prevented between health care provider and PWB. It was quite necessary to identify communication barriers between pharmacist and person with blindness (PWB). Qualitative study was used to describe the pharmacist-PWB communication barriers. Selected respondents [pharmacists, pharmacy assistants, PWB] who live in Yogyakarta city (Indonesia) and met inclusion criteria were invited to join focus group discussion (FGD) and in-depth interview during February to April 2015. The data was analyzed using content analysis. Ten barriers that had been identified in this study were perceived in specific interaction during communication between PWB and pharmacists. Theoretically, those barriers compared with barriers that appeared between general patients and pharmacists are likely more complex. Barriers of communication that successfully identified are (1)medicine characteristics;(2)senses;(3)services to PWB;(4) memory;(5)medicine information delivery;(6)PWB characteristic related to medicine;(7)regulation;(8)emotion during communication;(9)soft skills in communication and (10)misconception about equality. It developed and theoretically established as map of communication barriers in order to prevent errors and achieve effective communication. Three possible recommendations that had been addressed are inviting PWBs trustee, revamping pharmacy systems, and improving pharmacy curriculum. It expected to minimize barriers and furthermore, it could achieve optimum goals in medication, fulfill equality and attain the highest standard of health outcome of PWB.
