ANALISIS PEMBIAYAAN KESEHATAN PROGRAM KESEHATAN IBU DAN ANAK (KIA) BERDASARKAN STANDAR PELAYANAN MINIMAL (SPM) DI KABUPATEN NUNUKAN

2015 | Tesis |Latar belakang: Peran pemerintah daerah dalam era desentralisasi sangat penting terhadap bidang kesehatan terutama dalam pembiayaan. Hal ini dikarenakan kesehatan merupakan salah satu bidang yang didesentralisasikan. SPM merupakan urusan wajib daerah yang berhak diperoleh setiap warga secara minimal dan salah satu pelayanan dasar adalah program KIA. Tujuan penelitian: Memperoleh gambaran kecukupan biaya program KIA berdasarkan hasil perhitungan SPM dan efektifitas kinerja pembiayaan program KIA dalam mencapai sasaran dan tujuan program. Metode penelitian: Penelitian ini dilakukan di Kabupaten Nunukan tahun 2013 dan merupakan penelitian deskriptif dengan data kuantitatif. Analisis kecukupan dilakukan dengan menghitung biaya program KIA yang tersedia terhadap hasil perhitungan berdasarkan SPM. Analisis efektifitas capaian kinerja program KIA terhadap target indikator nasional dan AKI/AKB terhadap target RPJMN 2010-2014. Hasil penelitian: Biaya program KIA yang tersedia Rp. 2.530.038.761 dimana jumlahnya lebih kecil dari hasil perhitungan biaya berdasarkan SPM yang berjumlah Rp. 3.707.719.364. Terdapat kekurangan biaya sebesar Rp 1.177.680.603. Pembiayaan untuk kegiatan langsung sebesar 74% sementara itu untuk kegiatan tidak langsung 26%. Persentase pembiayaan program KIA dari pemerintah pusat sekitar 75%. Kinerja pembiayaan program KIA belum efektif dalam mencapai target indikator SPM nasional dan AKI masih cukup tinggi 173 per 100.000 kelahiran hidup dan AKB yang berjumlah 14 per 1000 kelahiran hidup. Kesimpulan: Biaya program KIA tidak cukup untuk melaksanakan kegiatan program KIA terdapat kekurangan biaya sebesar Rp. 1.177.680.603 dan adanya ketergantungan pembiayaan program KIA terhadap pemerintah pusat sebesar 75,0% hal ini dikarenakan masih rendahnya komitmen pemerintah daerah terhadap program prioritas seperti program KIA. Tidak efektifnya kinerja pembiayaan program KIA terhadap sasaran dan tujuan program yang telah ditetapkan.
Background: The role of regional government in decentralization period is very important to the health sector particularly in financing. This is because health is one of sectors that was decentralized. Minimum service standard is obligation responsibility for the region that is the right for every citizens at minimum and one of many basic services is Maternal and Child Health programs. Objectives: Get depiction on the financing sufficiency for Maternal and Child Health Programs based on Minimum Service Standard cost calculation and the effectivity of performance financing of Maternal and Child Health programs to achieve program’s objectives and goals. Methods: This research is a descriptive study conducted in Nunukan in 2013 with quantitative data. sufficiency analysis is done by calculating the cost of Maternal and Child Health program to the result of cost calculation based on Minimum Service Standard. Performance effectivity of the Maternal and Child Health Programs was analyzed by the achievement of Minimum Service Standard to the national indicator target and Maternal Mortality and Infant Mortality to the target of RPJMN of 2010-2014. Results: The cost of Maternal and Child Health program that was available was Rp. 2.530.038.761, and the calculation result of the cost based on Minimum Service Standard at Rp. 3.707.719.364, thus there was fund deficit about Rp 1.177.680.603. The percentage of MCH programs financing by central government was still hight that was about 75,0%. There are financing MCH programs for type activities direct amounted to 74%, and indirect activities amounted to 26%. The financing performance of MCH program was not effective yet to achieve the target of national Minimum Service Standard indicator and maternal mortality was still high at 173 per 100.000 life birts and infant mortality at 14 per 1000 life births. Conclusion: The financing of MCH programs was not sufficient to perform activities in MCH programs, thus there was fund deficit about Rp 1.177.680.603.
