FAKTOR-FAKTOR PENYEBAB TIDAK TERSEDIANYA BERKAS REKAM MEDIS DI BAGIAN PENYIMPANAN REKAM MEDIS PADA SAAT PELAYANAN DI HAPPY LAND MEDICAL CENTRE YOGYAKARTA

Latar belakang: Rekam medis merupakan jantung dari sarana pelayanan kesehatan. Rekam medis sangat berpengaruh terhadap mutu pelayanan medis yang diberikan oleh rumah sakit beserta staff medisnya. Salah satu Unit Rekam Medis yang menunjang dalam pelayanan rumah sakit adalah unit penyimpanan berkas rekam medis. Dalam penyimpanan berkas rekam medis dapat terjadi salah letak (misfile) dan berkas rekam medis tidak ada saat dibutuhkan untuk pelayanan. Apabila hal ini terjadi, maka akan menghambat jalannya pelayanan pasien di rumah sakit. Berdasarkan studi pendahuluan di Happy Land Medical Centre Yogyakarta pada tanggal 2 November 2014 yang dilakukan melalui pengamatan selama jalannya pelayanan diketahui bahwa di unit penyimpanan berkas rekam medis terdapat berkas rekam medis yang tidak ditemukan pada saat proses pelayanan. Berdasarkan pengamatan rata-rata jumlah kunjungan pasien 150 orang per hari rasio persentase tidak ditemukan berkas rekam medis adalah 25 % - 30 %, artinya rata-rata berkas rekam yang tidak ditemukan adalah antara 37-45 berkas. Tujuan: tujuan penelitian adalah untuk mengetahui sistem pengelolaan di bagian penyimpanan berkas rekam medis di Happy Land Medical Centre Yogyakarta dan mengetahui faktor-faktor penyebab tidak tersedianya berkas rekam medis di bagian penyimpanan berkas rekam medis di Happy Land Medical Centre. Metode Penelitian: Jenis penelitian adalah penelitian deskriptif dengan pendekatan kualitatif. Rancangan penelitian yang digunakan adalah CrossSectional. Teknik pengumpulan data dengan wawancara dan observasi. Sumber data dalam penelitian ini adalah petugas rekam medis di Happy Land Medical Centre Yogyakarta, sarana, kondisi ruang dan aktivitas penyimpanan berkas rekam di Happy Land Medical Centre Yogyakarta, dan berkas rekam medis Happy Land Medical Centre Yogyakarta yang sudah tersimpan di rak penyimpanan Hasil: Berdasarkan lokasi penyimpanannya, sistem penyimpanan rekam medis yang digunakan di Happy Land Medical Centre Yogyakarta adalah sentralisasi. Sedangkan sistem penjajaran berkas rekam medis di Happy Land Medical Centre Yogyakarta menggunakan sistem angka akhir (terminal digit filing). Faktor-faktor penyebab tidak tersedianya berkas rekam medis di bagian penyimpanan rekam medis pada saat pelayanan adalah salah simpan (misfile) dikarenakan terbatasnya petugas penyimpanan/penjajaran dan kurangnya ketelitian petugas, berkas rekam medis masih di poliklinik dikarenakan masih dalam proses rekap di poliklinik oleh perawat dan berkas rekam medis masih tertinggal di bangsal dikarenakan perawat yang masih baru yang tidak langsung mengembalikan berkas rekam medis ke bagian penyimpanan berkas rekam medis

Background: Medical records are at the heart of health-care facilities. Medical records affects the quality of medical services provided by hospitals and their medical staff. One of Medical Record Unit that support the hospital services is medical record file storage unit. In medical record file storage can occur misplaced (misfile) and the medical record file does not exist when it needs to service. If this is the case, then it would impede the course of patient care in hospitals. Based on preliminary studies in Happy Land Medical Centre Yogyakarta on November 2, 2014 is done through observation during the course of the service it is known that in the storage unit medical record file contained medical record file that was not found during the service process. Based on observations of the average number of patient visits to 150 people per day percentage ratio found no medical record file is 25% - 30%, meaning that the average record file that is not found is between 37-45 files. Objective: the purpose of the study is to determine the management system in the medical record file storage in the Happy Land Medical Centre Yogyakarta and determine the factors causing the unavailability of medical record file in the file storage of medical records in the Happy Land Medical Centre. Methods: The study was a descriptive study with qualitative approach. The design of the study is cross-sectional. Data collection techniques with interviews and observations. Source of data in this study is a medical records clerk at Happy Land Medical Centre in Yogyakarta, the means, conditions of storage space and activity record file in the Happy Land Medical Centre in Yogyakarta, and medical record file of Happy Land Medical Centre Yogyakarta that has been stored in the storage rack. Results: Based on the location of its storage, record-keeping systems used in the Happy Land Medical Centre Yogyakarta is centralization. While the medical record file system alignment in the Happy Land Medical Centre Yogyakarta used the final number system (terminal digit filing). Factors that cause the unavailability of medical record file in the storage of medical records at the time the service is to keep (misfile) due to lack of officers storage / alignment and lack of rigor officers, medical record file is still in the clinic due to still in the process of recap in the clinic by nurses and medical record file remains on the ward because nurses are still new, not directly restore files to the storage of medical records medical record file.
