NILAI AKURASI DIAGNOSTIK DELTA NEUTROPHIL INDEX (DNI) SEBAGAI PENANDA DERAJAT KEPARAHAN SEPSIS

Latar belakang: Angka kematian pada sepsis masih tinggi. Penyebab utama kematian terutama akibat disfungsi atau kegagalan akut multi organ yang progresif. Penilaian disfungsi organ dapat memberikan informasi penting dalam mengambil keputusan klinis. Granulosit imatur merupakan salah satu parameter yang tersedia untuk mendeteksi sepsis. Delta Neutrophil Index (DNI) merupakan nilai yang menggambarkan fraksi granulosit imatur di sirkulasi darah. Tujuan Penelitian: untuk mengetahui nilai area under receiver-operating characteristics curve (AUROC) dari DNI sebagai penanda derajat keparahan sepsis pada pasien sepsis di RSUP Dr. Sardjito Yogyakarta. Metode Penelitian: menggunakan desain potong lintang yang diadakan Juni sampai dengan Desember 2014. Subyek adalah pasien dengan klinis sepsis. Pasien sepsis dikelompokkan menurut derajat keparahannya yaitu sepsis dan sepsis berat/syok sepsis kemudian dilakukan pemeriksaan DNI. Analisa data dilakukan dengan program komputer uji statistik. Nilai akurasi diagnostik DNI diperoleh dari analisa kurva ROC untuk menentukan nilai AUC. Analisa diagnostik lain meliputi sensitivitas, spesifisitas, NPV, PPV dan likelihood ratio juga dilakukan terhadap beberapa nilai cut-off DNI. Nilai cut-off optimal DNI diperoleh melalui Youden index. Hasil Penelitian: dari 63 subyek sepsis diperoleh 37 orang sepsis berat/syok sepsis dan 26 orang sepsis. Rerata usia subyek 52,11 +- 14,56 tahun. Rerata DNI adalah 16,76 +- 6,29% pada pasien sepsis berat/syok sepsis dan 5,23 +- 2,42% pada pasien sepsis. Nilai AUC DNI adalah 97,4% (95% CI: 0,899-0,998, p < 0,0001). Nilai cutoff DNI yang dapat digunakan adalah 7,8% dengan sensitivitas 97,3(95% CI: 85,899,9)%, spesifisitas 84,6(95% CI: 65,1-95,6)%, NPV 95,7(95% CI: 77,4-99,9)%, PPV 90(95% CI: 76,3-97,2)%, +LR 6,3(95% CI: 2,6-15,6) dan -LR 0,03(95% CI: 0,005-0,2). Kesimpulan: Nilai akurasi diagnostik AUC dari DNI adalah 97,4% sebagai penanda sepsis berat/syok sepsis yang sangat baik.

Background: Mortality rate of sepsis is still quite high. The main causes of death are due to organ dysfunction or multi organ failure. Early organ dysfunction identification may important in clinical decision making. Immature granulocyte is one of the parameters available for sepsis identification. DNI describes the fraction of immature granulocyte in circulation. DNI showed as a pretty good of early biomarker of sepsis. The advantages are easily available and relatively cheap price. Objective: to determine the area under receiver-operating characteristic curve (AUROC) value of DNI as a biomarker of disease severity in patients with sepsis in the Hospital Dr. Sardjito. Methods: This study used cross-sectional designs and subjects were patients with clinically sepsis, held between June to December 2014. Patients were grouped according to the severity of sepsis as sepsis and severe sepsis/septic shock. All subjects were examined for DNI. The data were analyzed with statistical computer programs. The diagnostic accuracy value was conducted with AUROC analysis. The sensitivity, spesificity, NPV, PPV and likelihood ratio test were also performed. The DNI optimal cut-off value was obtained through Youden index.. Results: of 63 subjects were obtained that 37 severe sepsis/septic shock and 26 sepsis patients. Patients mean age was 52,11 +- 14,56 years. The mean of DNI was 16,76 +- 6,29% in severe sepsis/septic shock and 5,23 +- 2,42% in sepsis. The AUC value of DNI was 97,4% (95% CI: 0,899 to 0,998; p < 0,0001). The DNI optimal cut-off that can be used is 7,8% with sensitivity 97,3(95% CI: 85,8-99,9)%, spesificity 84,6(95% CI: 65,1-95,6)%, NPV 95,7(95% CI: 77,4-99,9)%, PPV 90(95% CI: 76,3-97,2)%, +LR 6,3(95% CI: 2,6-15,6) and -LR 0,03(95% CI: 0,005-0,2). Conclusions: The AUC value of DNI was 97,4% as an excellent biomarker of severe sepsis/septic shock.
