Latar Belakang : Rekam Medis adalah berkas yang berisi catatan dan dokumen antara lain identitas pasien, hasil pemeriksaan, pengobatan yang telah diberikan, serta tindakan dan pelayanan lain yang telah diberikan kepada pasien. Akreditasi rumah sakit adalah pengakuan terhadap rumah sakit yang diberikan oleh lembaga independen penyelenggara Akreditasi yang ditetapkan oleh Menteri, setelah dinilai bahwa rumah sakit itu memenuhi standar pelayanan rumah sakit yang berlaku untuk meningkatkan mutu pelayanan rumah sakit secara berkesinambungan Peningkatan kualitas pelayanan sangatlah penting agar rumah sakit mampu berkompetisi baik di tingkat regional, nasional bahkan Internasional. Dimana untuk mencapai pengakuan dari tim akreditasi maka rumah sakit perlu melakukan berbagai macam persiapan yang termasuk salah-satunya menyiapkan berbagai dokumen yang lengkap khususnya dokumen rekam medis sebagai bukti pelaksanaan pelayanan. Rumah Sakit Bethesda Lempuyangwangi sedang dalam proses persiapan akreditasi. Maka perlu dilakukan penilaian pada dokumen tersebut. Tujuan : Mengetahui sejauh mana Kesiapan Kelengkapan dokumen Terkait Rekam Medis Sesuai Pelaksanaan Elemen Penilaian Pada sasaran keselamatan pasien yang berfokus pada identifikasi pasien di Rumah Sakit dalam standar akreditasi 2012 di Rumah Sakit Umum Bethesda Lempuyangwangi. Metode Penelitian : Jenis penelitian yang digunakan dalam penelitian ini adalah penelitian deskritif dengan pendekatan kualitatif. Sampel subjek dalam penelitian ini yaitu Kepala Rekam Medis, Koordinator Persiapan Akreditasi, petugas Rekam Medis, petugas laboratorium, petugas perawat dan petugas farmasi. Sampel objek dari penelitian ini yaitu berkas Rekam Medis rawat inap yang diambil sebanyak 30 berkas dan SPO. Hasil Penelitian : Dari elemen penilaian yang berkaitan dengan rekam medis dalam identifikasi pasien sudah dilengkapi dengan kebijakan serta pedoman pelaksanaan, namun masih terdapat dokumen pelaksanaan pelayanan yang belum ada serta dalam pendokumentasian belum konsisten khususnya dalam identifikasi pasien. Hospital accreditation is recognition of the hospital given by the independent Accreditation organizers stipulated by the Minister, after the hospital assessed that it meets the standard of hospital services applicable to improve the quality of hospital services on an ongoing basis Improved quality of service is very important that the hospital is able to both competed at the regional, national and even international. Where to achieve recognition of the hospital accreditation team needs to perform a wide variety of preparations which include one of the only complete preparing various documents, especially document the medical record as proof of service implementation. Lempuyangwangi Bethesda Hospital is in the process of preparation for accreditation. It is necessary to vote on the document. Objective: To determine the extent of readiness Documentation Related to Medical Record Implementation Element Assessment In accordance patient safety goals focused on the identification of patients at the Hospital of the accreditation standards in 2012 at the General Hospital Bethesda Lempuyangwangi. Methods: This type of research used in this research is descriptive research with a qualitative approach. Isubjects in the study sample was Chief Medical Record, Accreditation Preparation Coordinator, Medical Record officers, laboratory workers, nurses and pharmaceutical personnel. Samples object of this research is the Medical Record file hospitalization taken as many as 30 files and SPO. Results: Of the elements of assessment relating to the identification of the patient's medical record is equipped with the policies and guidelines for the implementation, but there is still a service implementation document that does not exist and the documentation has not been consistent, especially in the identification of patients.
