Latar Belakang: Desain pada formulir Klinik Gigi terdapat beberapa kekurangan. Diantaranya dilihat dari aspek anatomi, heading dalam desain pada formulir Klinik Gigi di Rumah Sakit Slamet Riyadi Surakarta belum mencantumkan nomor halaman, nomor edisi dan nomor revisi. Pada body, margin yang digunakan belum sesuai standar. Sedangkan dilihat dari rules yang digunakan, dalam formulir Klinik Gigi di Rumah Sakit Slamet Riyadi Surakarta masih terlihat tidak teratur. Dari aspek isi dapat dilihat bahwa item yang terdapat dalam formulir tersebut belum lengkap. Masih terdapat beberapa item data yang perlu dicantumkan dalam formulir Klinik Gigi yang belum termuat di dalamnya. Selain itu, desain pada formulir tersebut dirasa belum sesuai, belum memenuhi kebutuhan pengguna, sehingga dibutuhkan pembenahan.serta belum terdapatnya persetujuan tindakan gigi padahal persetujuan tindakan gigi sangat penting untuk bukti secara tertulis bahwa pasien tersebut menyetujui tindakan yang akan dilakukan Tujuan: Merancang formulir Klinik Gigi dan Persetujuan Tindakan Gigi Proses Perancangan: Perancang melakukan studi pendahuluan di Rumah Sakit Slamet Riyadi Surakarta dan menentukan akan melakukan desain ulang formulir Klinik Gigi dan desain persetujuan Tindakan gigi. Kemudian dilakukan identifikasi formulir yang telah ada di Rumah Sakit Slamet Riyadi Surakarta dengan wawancara kepada responden. Setelah mengetahui hal yang perlu diperbaiki, perancang membuat Dua alternatif desain formulir Klinik gigi dan persetujuan tindakan gigi dan diajukan kepada responden untuk dipilih dan diberi masukan. Desain terpilih dilakukan revisi dengan memperhatikan masukan dari responden. Setelah revisi, perancang menunjukkan kembali hasil desain terpilih yang telah direvisi kepada responden untuk disetujui. Hasil: Terdapat beberapa item data dalam Formulir Klinik gigi yang harus dipertahankan, ditambahkan, maupun dihilangkan guna mencapai tujuan formulir.serta mendesain persetujuan tindakan gigi Dan Desain terpilih untuk formulir Klinik gigi yang telah direvisi menggunakan kertas HVS 80 gram ukuran A4 dengan background putih warna font hitam, serta rules dan logo kuning., nama rumah sakit dengan ukuran font 13 point, alamat, nomor telepon, dengan ukuran font 11 point, serta judul formulir dengan ukuran font 12 point yang terletak di tengah atas formulir, sedangkan font pada body berukuran 10 point. Margin yang digunakan adalah 1 cm pada bagian atas, 1,3 cm pada bagian bawah, 2 cm pada bagian samping kiri, dan 1 cm pada samping kanan. Formulir Klinik gii terdiri dari dua halaman. Sedangkan untuk formulir persetujuan tindakan gigi yang telah direvisi menggunakan kertas HVS 80 gram ukuran A4 dengan background putih warna font hitam, serta rules dan logo kuning., nama rumah sakit dengan ukuran font 12 point, alamat, nomor telepon, dengan ukuran font 12 point, serta judul formulir dengan ukuran font 13 point yang terletak di tengah atas formulir, sedangkan font pada body berukuran 10 point. Margin yang digunakan adalah 0,8 cm pada bagian atas, 1,3 cm pada bagian bawah, 2,5 cm pada bagian samping kiri, dan 1 cm pada samping kanan. Background: There are some shortcomings in the design of the form Dental Clinic. And there are not the presence of informed consent when consent gear teeth is very important for proof in writing that the patient agree actions to be taken Objective: Designing form Dental Clinic and Dental Measures Agreement Process Design: The designer did a preliminary study in Slamet Riyadi Hospital Surakarta and decisive to redesign form Dental Clinic and dental Measures design approval. And then identifying existed form in Slamet Riyadi Hospital Surakarta with interviews to the respondent. After knowing the things that need to be corrected, the designer create two alternative designs and consent form dental clinic and dental action, then submitted to the respondent to be selected and given input. The selected design revised with input from respondents. After the revision, the designer showed again chosen design results revised to respondents for approval. Results: There were several items of data in the form of dental clinics that should be retained, added to, or removed to achieve the goal form. As well as designing the consent of teeth and Design chosen to form dental clinic that has been revised using the HVS 80 gram A4 size with a white background and black font color, with rules and yellow logo, The name of the hospital with 13 point font size, address, telephone number, with 11 point font size, and form with the title font size 12 point located in the middle of the top of the form, while the body font size 10 point.
