Rancangan Berkas Rekam Medis di Puskesmas Trucuk II

Latar Belakang: Salah satu fungsi penyimpanan berkas rekam medis adalah untuk mempermudah dan mempercepat penyediaan berkas rekam medis yang disimpan dalam rak filing. Di Puskesmas Trucuk II masih terdapat formulir rekam medis yang hilang, sobek dan tercecer. Kesulitan dalam pencarian berkas karena nomer rekam medis tidak terlihat jelas dikarenakan nomer rekam medis berada diatas kop map akibatnya nomor rekam medis tidak terlihat saat berkas berada dalam rak penyimpanan. Bahan map memakai kertas dupleks karton tipis dan manila tipis yang masih mudah rusak saat digunakan sebagai map berkas rekam medis. Pada Puskesmas Trucuk II formulir rawat jalan poliklinik umum masih banyak kekurangan maka dari itu perlu dilakukan perancangan ulang formulir rawat jalan poliklinik umum. Karena di Puskesmas Trucuk II belum mempunyai formulir rawat jalan poliklinik gigi maka perlu juga dilakukan perancangan pada formulir gigi agar data yang dihasilkan lebih akurat. Tujuan Umum: Rancangan ditujukan untuk memperbaiki formulir dasar di Puskesmas Trucuk II agar lebih spesifik lagi sesuai dengan kebutuhan setiap poliklinik dan membuat map rekam medis dengan memperhatikan pertimbangan khusus desain formulir kertas, pertimbangan khusus konstruksi formulir kertas, dan pertimbangan konten isi setiap formulir sehingga memudahkan pengisian, pencarian data, penyimpanan berkas, dan pengambilan berkas kembali. Diantaranya merancang formulir rawat jalan poliklinik umum, formulir rawat jalan poliklinik gigi dan map berkas rekam medis. Tujuan Khusus: Merancang formulir rawat jalan poliklinik umum Puskesmas Trucuk II, merancang formulir rawat jalan poliklinikgigi Puskesmas Trucuk II, dan merancang map (folder) berkas rekam medis Puskesmas Trucuk II. Proses Pembuatan Rancangan: Diawali dengan mengumpulkan data melalui observasi, wawancara I untuk mengetahui item-item apa saja yang dibutuhkan dan WawancaraII untuk mengetahui desain formulir rawat jalan poliklinik umum, formulir rawat jalan poliklinik gigi dan map berkas rekam medis yang seperti apa yang sesuai dengan keadaan di Puskesmas Trucuk II dan dibantu dengan teori pendukung rancangan formulir rawat jalan polkilinik umum, formulir rawat jalan polkilinik gigi dan map berkas rekam medis. Membuat dua rancangan alternatif formulir rawat jalan polkilinik umum, formulir rawat jalan polkilinik gigi dan map berkas rekam medis untuk dipilih salah satu yang paling sesuai dengan keadaan dan kebutuhan. Hasil: Rancangan formulir rawat jalan poliklinik umum pada alternatif pertama, rancangan formulir rawat jalan poliklinik gigi pada alternatif pertama dan rancangan map (folder) berkas rekam medis pada alternatif pertama

Background:One of the medical record file storage function is to facilitate and accelerate the provision of medical record file stored in filing shelves. The Public Health Center Trucuk II is still a form of medical records were missing, torn and scattered. Difficulties in the search for the file number of medical records is not visible due to medical record number is above the header folders consequently medical record number is not visible when the files are in a storage rack. Duplex paper binder material wears thin cardboard and thin manila still vulnerable to damage when used as a medical record file folder. At the health center Trucuk II common form of outpatient polyclinics are still many short comings there for enecessary to redesign medical record form general outpatient clinic. Because in Public Health Center Trucuk II does not have any form of outpatient dental clinic it is necessary also to design the tooth form so that the resulting data is more accurate. General Purpose:The design is intended to improve the basic form in Public Health Center Trucuk II in order to more specifically according to the needs of each clinic and make a folder of medical records by taking into account special considerations paper form design, special consideration construction paper form, and consideration of the contents of each form and content so as to facilitate charging, data search, file storage, and retrieval of files back. Among the outpatient clinic designing common form, form outpatient dental clinic and a medical record file folder. Specific Objectives: Designing a common form of outpatient polyclinic Public Health Center Trucuk II, designed the form of out patient health center dental clinicTrucuk II, and designing folders to Public Health Center medical record fileTrucuk II. Process design: Starting with collecting data through observation, interviews it know what items are needed and interview to determine the form and design of general outpatient clinic, outpatient dental clinic form and medical record file folders like what suits the state of the Public Health Center Trucuk II and assisted with supporting the outpatient policlinic draft general form, form policlinic outpatient dental and medical record file folder. Make two alternative designs policlinic outpatient general form, form policlinic outpatient dental and medical record file folder to select the one most appropriate to the circumstances and needs. Results: The design form a general outpatient clinic ont he first alternative, the draft form of outpatient dental clinic on the first alternative and the draft folder medical record file on the first alternative.
