PENDOKUMENTASIAN LEMBAR PERSETUJUAN TINDAKAN MEDIS (INFORMED CONSENT) DI KLINIK GIGI FRESH DENTAL

Latar Belakang: Dalam rekam medis terdapat suatu lembar atau formulir yang berisikan pernyataan persetujuan dari pasien atau keluarga pasien untuk dilakukan suatu tindakan medis atau dalam dunia medis dikenal sebagai formulir Informed Consent.Ketidaklengkapan formulir Informed Consent dapat memberikan dampak negatif bagi proses pelayanan kesehatan pasien karena dapat mengakibatkan tuntutan hukum yang dapat merugikan pasien, rumah sakit atau tenaga kesehatan itu sendiri. Berdasarkan studi pendahuluan yang dilakukan oleh peneliti menemukan bahwa Klinik Praktek Bersama Dokter Gigi Fresh Dental mempunyai lembar persetujuan tindakan medis (informed consent). Namun pada praktek dilapangan, peneliti menemukan bahwa lembar informed consent tersebut tidak digunakan.Semua informasi atau data dari pemberi informasi dan dari pemberi persetujuan dituliskan di lembar rekam medis gigi. Tujuan:Mengetahui penulisan atau pengisian lembar persetujuan tindakan medis (informed consent) dan penyimpanan lembar persetujuan tindakan medis (informed consent). Metode Penelitian: Jenis penelitian ini adalah penelitian deskriptif dengan pendekatan kualitatif dengan rancangan penelitian retrospektif. Pengumpulan data menggunakan teknik wawancara, observasi dan studi dokumentasi. Hasil: Pelaksanaan pendokumentasian lembar persetujuan tindakan medis (informed consent) dilakukan di Klinik Praktek Bersama Dokter Gigi Fresh Dental. Penulisan atau pengisian lembar persetujuan tindakan medis dituliskan pada lembar rekam medis gigi, sedangkan lembar informed consent itu sendiri tidak digunakan. Tidak digunakannya lembar informed consent karena melakukan penghematan kertas dan menghindari hilangnya lembar informed consent itu sendiri. Penyimpanannya berdasarkan alfabetik atau abjad nama pasien dan disimpan secara bertumpuk. Berkas rekam medis disimpan di dalam lemari penyimpanan, dimana pada rak terakhir digunakan untuk menyimpan obat-obatan dan barang-barang yang diperlukan untuk kegiatan pengobatan diklinik tersebut.

Methods: The study was a descriptive study with qualitative approach with the retrospective study design. Collecting data using interview, observation and documentation study.
