Latar Belakang : Klinik & Rumah Bersalin Harapan Ibu merupakan instansi pelayanan kesehatan yang berfokus pada kasus persalinan / obstetrik dan beberapa pelayanan kesehatan lainnya. Formulir rekam medis obstetrik di klinik tersebut sudah ada tetapi belum memenuhi kebutuhan pengguna, sehingga perlu dilakukan redesain / perancangan ulang terhadap formulir rekam medis obstetrik, agar dapat memenuhi kebutuhan pengguna serta sesuai dengan standar yang berlaku. Tujuan : Merancang ulang formulir obstetrik yang digunakan sebagai sumber informasi yang lengkap dengan menekankan pada aspek isi, fisik serta anatomik sehingga dapat digunakan sebagai media pengumpulan data yang lengkap dan dapat digunakan bagi pihak-pihak yang berkepentingan. Proses Pembuatan Rancangan : Proses pembuatan rancangan diawali dengan penentuan ide rancangan, penentuan teori pendukung perancangan, identifikasi kebutuhan formulir obstetrik, pembuatan 2 alternatif, pemilihan rancangan alternatif, perbaikan rancangan alternatif terpilih, perwujudan alternatif terpilih dan hasil rancangan. Hasil : Rancangan terpilih yaitu formulir rekam medis obstetrik alternatif kedua yang terdiri dari lima formulir yaitu satu lembar data ibu hamil, tiga lembar ringkasan persalinan dan satu lembar data bayi baru lahir. Ukuran formulir adalah A4 21 x 29,7 cm, menggunakan kertas HVS 80 gr, warna identitas klinik, body, huruf item dan huruf pada isian item data adalah berwarna hitam.

Background : Klinik & Rumah Bersalin Harapan Ibu is a health care institution that focuses on the case of delivery / obstetric and several other health services. Form obstetric medical records at the clinic already exists but do not meet the needs of users, so it is necessary to re-design / re-design of the form obstetric medical records, in order to meet user requirements and in accordance with applicable standards. Aim : Redesigning obstetric form used as a complete source of information with an emphasis on the content aspect, the physical and anatomical so that it can be used as a medium for data collection is complete and can be used for the parties concerned. Process Design : The design process begins with determining the design ideas, determination theory supporting the design, form of identification needs obstetric, creation of two alternatives, the selection of alternative designs, alternative design fixes selected, an alternative embodiment chosen and the design. Result : Selected draft form of medical records is obstetric second alternative form which consists of five pieces of data, namely a pregnant woman, three- sheet summary of birth and a data sheet newborns. Form A4 size is 21 x 29.7 cm, using the HVS 80 gr, color identity clinic, body, letters items and letters on the field data item is black.
