LatarBelakang: Metode penulisan catatan perkembangan yang menjelaskan tiga hal yaitu apa yang telah terjadi dengan pasien; apa yang direncanakan untuk pasien; dan bagaimana pasien bereaksi terhadap terapi, ditempuh 4 langkah tentang proses pengambilan keputusan secara sistematis yang dikenal dengan sebutanSOAP. Berdasarkan studi pendahuluan yang dilakukan peneliti dengan studi dokumentasi dan wawancara dengan kepala rekam medis di instalasi rekam medis RSPAU dr. S. Hardjolukito diketahui bahwa masih terdapat ketidakkonsistenan dalam pencatatan pada formulir catatan perkembangan terintegrasi pasien rawat jalan sesuai SOAP. Tujuan: Mengetahui persentase kelengkapan dan ketidaklengkapan pengisian formulir catatan perkembangan terintegrasi pasien rawat jalan sesuai SOAP dan Mengetahui faktor-faktor penyebab ketidaklengkapan pengisian formulir catatan perkembangan terintergrasi pasien rawat jalan sesuai SOAP di RSPAU dr. S. Hardjolukito. MetodePenelitian: Jenis penelitian adalah penelitian deskriptif dengan pendekatan kualitatif. Rancangan penelitian yang digunakan adalah studi kasus. Subjek dalam penelitian ini adalah 2 perawat poliklinik dan kepala instalasi rekam medis. objek dalam penelitian ini adalah 100 berkas rekam medis pasien rawat jalan.Teknik pengumpulan data dengan wawancara dan studi dokumentasi. Untuk validasi data menggunakan triangulasi sumber. Penelitian ini dilakukan di ruang instalasi rawat jalan dan instalasi rekam medis. Hasil: berdasarkan hasil analisis dari 100 berkas rekam medis pasien rawat jalan yang didalamnya terdapat formulir catatan perkembangan terintegrasi pasien rawat jalan diperoleh hasil persentase kelengkapan pencatatan sesuai SOAP sebesar 58% dan pencatatan tidak lengkap sesuai SOAP sebanyak 42%. Faktor yang menyebabkan ketidaklengkapan pengisian adalah kesibukan individu yang mengisikan formulir catatan perkembangan terintegrasi pasien rawat jalan dikarenakan banyaknya pekerjaan, belum adanya item khusus untuk pengisian tiap komponen SOAP dan juga belum terdapatnya Standar Operasional Prosedur (SOP) tentang pencatatan pada formulir catatan perkembangan terintegrasi pasien rawat jalan sehingga menyebabkan ketidaklengkapan dalam pencatatan pada formulir catatan perkembangan terintegrasi pasien rawat jalan. Background: the method of writing a note that explains the development of the three things that is what has happened with the patient; What is planned for the patient; and how the patient reacts to therapy, is 4 steps of the decision-making process in a systematic, known as SOAP. Based on a preliminary study conducted by researchers of the study documentation and interviews with the head of the medical record in the installation medical record RSPAU Dr. s. Hardjolukito noted that there are still inconsistencies in record-keeping on the integrated development of the entry form outpatients according to SOAP. Objectives: This research aims to know the percentage of completeness and incompleteness form of develompent notes of integrated outpatient appropriate SOAP and find out the factors that causes of incomplete form of development notes of integrated outpatient according to SOAP at RSPAU Dr. s. Hardjolukito. Methods: this type of research is descriptive research with qualitative approach. The research design used was a case study. The subject in this study was 2 nurse clinic and head installation medical record. the object in this research are 100 files outpatient medical record. For collecting data using interviews and documentation study. For validation of data using the triangulation of sources. This research was done in the space of the installation and the installation of outpatient medical record. Results: based on the results of the analysis to 100 file outpatient medical record which contained an integrated development entry form outpatient retrieved results percentage of completeness of recording of corresponding SOAP of 58% and incomplete record-keeping appropriate SOAP as much as 42%. Factors that cause incomplete charging is the flurry of individuals who filled in the form a record of the development of integrated outpatient because of the large number of jobs, not to the existence of special items for completing each component and also a SOAP yet there is a Standard Operational Procedure (SOP) of the recording on the form notes the development of integrated outpatient causing incompleteness in a record on a form recording the development of integrated outpatient.
