ANALISIS KELENGKAPAN FORMULIR TRANSFER PASIEN TERKAIT PERSIAPAN AKREDITASI KARS 2012 DI RSJ GRHASIA YOGYAKARTA

Latar Belakang: Berdasarkan studi pendahuluan yang dilakukan peneliti pada bulan Maret 2015 terhadap 39 formulir transfer pasien, ditemukan angka keterisian dengan persentase 65% untuk kelompok item identitas pasien, 61% untuk kelompok item situasi, 31% untuk kelompok item latar belakang, 54% untuk kelompok item assessment, 40% kelompok item rekomendasi, dan 81% kelompok item autentikasi. Angka ini termasuk angka keterisian yang cukup rendah bagi sebuah pelayanan rumah sakit mengingat formulir ini digunakan untuk melihat kondisi pasien pada saat dipindahkan. Tujuan: Penelitian ini bertujuan mengetahui kelengkapan item-item formulir transfer pasien berdasarkan standar MKI.8 akreditasi KARS 2012 dan mengetahui ketercapaian suatu elemen penilaian (EP) dari standar MKI.8 pada formulir transfer pasien di RSJ Grhasia Yogyakarta. Metodologi Penelitian: Jenis penelitian yang digunakan adalah deskriptif pendekatan kualitatif dengan rancangan penelitian cross sectional. Teknik pengumpulan data dengan menggunakan wawancara, observasi, dan studi dokumentasi. Untuk validitas data menggunakan triangulasi sumber. Hasil: Item formulir transfer pasien di RSJ Grhasia sudah sesuai dengan tujuh komponen elemen penilaian MKI.8. Persentase kelengkapan pada komponen alasan masuk rawat inap sebesar 99%, temuan signifikan sebesar 53%, diagnosis ditegakkan sebesar 75%, tindakan yang dilakukan sebesar 40%, obat-obatan persentase kelengkapannya sebesar 86%, kondisi pasien saat dipindah sebesar 98%. Untuk persentase kelengkapan pengisian tertinggi yaitu pada komponen alasan masuk rawat inap sebesar 99%. Sedangkan persentase kelengkapan terendah yaitu komponen tindakan yang dilakukan sebesar 40%. Jadi, total persentase keseluruhan dari EP MKI.8 yaitu 75% lengkap, 4% tidak lengkap, dan 21% tidak terisi. Berdasarkan persentase kelengkapan formulir transfer pasien yang lengkap sebesar 75%, sehingga mendapat skor 5 (lima) dan tercapai sebagian (TS).
