PERBEDAAN KETEBALAN RETINAL NERVE FIBER LAYER PERIPAPIL PADA ANAK, DEWASA DAN ORANG TUA NORMAL DENGAN MENGGUNAKAN STRATUS OPTICAL COHERENCE TOMOGRAPHY DI RSUP DR. SARDJITO YOGYAKARTA INDONESIA

2015 | Tesis | SP ILMU PENYAKIT MATATujuan : Mengetahui perbedaan ketebalan Retinal Nerve Fiber Layer (RNFL) peripapil pada anak, dewasa, dan orang tua normal. Metode : Desain penelitian berupa cross sectional dengan metode consecutive sampling. Subyek dibagi dalam 3 kelompok berdasarkan usia, masing-masing 30 orang, yaitu kelompok anak (8-18 tahun), dewasa (30-40 tahun) dan orang tua (50-60 tahun). Ketebalan RNFL kedua mata diukur dengan Optical Coherence Tomography (OCT) Stratus menggunakan protokol fast RNFL. Hasil diambil yang memiliki signal strength terbaik, minimal 7. Perbedaan rerata ketebalan RNFL, ketebalan RNFL per kuadran dan sektor antara 3 kelompok dianalisa dengan menggunakan one way anova. Hubungan antara usia dan ketebalan RNFL diukur dengan korelasi Pearson. Hasil : Rerata ketebalan RNFL pada penelitian ini untuk mata kanan/mata kiri (OD/OS) 104,88 ¼m / 106,94 ¼m. Rerata ketebalan RNFL kelompok anak, dewasa, dan orang tua untuk OD/OS berturut-turut 107.89 ¼m/109.50 ¼m, 107.28 ¼m/109.69 ¼m, dan 99.46 ¼m/101.63 ¼m. Ketebalan RNFL sesuai aturan ISNT. Analisa ANOVA pada ketiga kelompok usia mendapatkan perbedaan signifikan dengan p=0.001. Perbedaan signifikan tampak pada kuadran temporal, sektor 7-11. Perbandingan antara kelompok anak dan dewasa menunjukkan tidak ada perbedaan signifikan (OD p = 0.956; OS p = 0.996). Perbedaan signifikan terlihat pada perbandingan ketebalan RNFL pada anak dan dewasa dengan orang tua (p=0.004). Terdapat korelasi negatif antara usia dengan rerata ketebalan RNFL pada OD (r=-0.407; p=0.000) dan OS (r=-0.346; p=0.001). Kesimpulan : Terdapat perbedaan rerata ketebalan RNFL antar kelompok anak, dewasa, dan orang tua. Perbandingan rerata ketebalan antar kelompok menunjukkan perbedaan signifikan antara kelompok anak dengan orang tua dan dewasa dengan orang tua, di mana rerata ketebalan RNFL pada orang tua lebih tipis. Penelitian ini mendapatkan adanya korelasi negatif antara usia dengan rerata ketebalan RNFL.
Purpose: To know the differences of peripapillary Retinal Nerve Fiber Layer (RNFL) thickness in normal children, adults, and the elderly. Methods: This was a cross sectional study with consecutive sampling method. The subjects were divided into 3 groups based on age (30 people for each group), children (8-18years), adults (30-40years) and elderly (50-60years). RNFL thickness of both eyes was measured by Stratus Optical Coherence Tomography (OCT) using fast RNFL protocol. Results with the best signal strength, at least 7, were taken. The differences in mean RNFL thickness, RNFL thickness by quadrant and sector between the 3 groups were analyzed using one-way ANOVA. The relationship between age and RNFL thickness was measured by Pearson correlation. Result: The mean RNFL thickness in this study for the right eye / left eye (OD/OS) 104.88¼m/106.94 ¼m. The mean RNFL thickness in children, adults, and elderly groups for OD/OS were consecutively 107.89¼m/109.50¼m, 107.28¼m/109.69¼m, and 99.46¼m/101.63 ¼m. The RNFL thickness was in accordance with "ISNT" rule. ANOVA analysis for all groups showed significant differences with p=0.001. Significant differences appear in the temporal quadrant, sector 7-11. Comparisons between groups of children and adults showed no significant difference (OD p=0956; OS p=0.996). Significant differences were seen in the comparison of RNFL thickness in children and adults with elderly (p=0.004). There was a negative correlation between age and mean RNFL thickness, OD (r=-0407; p=0.000) and OS (r=-0346; p=0.001). Conclusions: There are differences between the mean RNFL thickness between groups of children, adults, and the elderly. The average thickness of the inter-group comparison showed a significant difference between the groups of children with parents and adults to the elderly, where the average RNFL thickness thinner in older people. This research obtains negative correlation between age and mean RNFL thickness.
