COST OF ILLNESS CHRONIC KIDNEY DISEASE (CKD) DENGAN TINDAKAN HEMODIALISIS DI RUMAH SAKIT BETHESDA YOGYAKARTA

Hemodialisis (HD) atau cuci darah sangat berperan penting bagi penderita gagal ginjal. Proses hemodialisis merupakan tindakan pengobatan yang mahal dan akan menjadi beban berat bagi pasien yang melakukan tindakan hemodialisis berulang kali selama seumur hidupnya. Tujuan penelitian ini adalah mengetahui total biaya penyakit Chronic Kidney Disease (CKD) dan untuk mengetahui perbedaan faktor pasien, faktor penyakit, dan faktor jenis pembiayaan terhadap biaya medik langsung pada pasien CKD dengan tindakan hemodialisis rawat jalan dan rawat inap di Rumah Sakit Bethesda Yogyakarta. Jenis penelitian ini menggunakan rancangan penelitian deskriptif analitik dengan pendekatan cross-sectional study menurut perspektif rumah sakit. Pengambilan data dilakukan secara retrospektif dengan melihat rekam medis pasien yang melakukan hemodialisis pada periode Januari sampai dengan Juni 2014. Variabel terikat (dependent variable) dalam penelitian ini adalah total biaya medik langsung pasien rawat inap dan rawat jalan yang melakukan tindakan hemodialisis di Rumah sakit Bethesda Yogyakarta, sedangkan variabel bebas (independent variable) adalah faktor pasien, faktor penyakit, dan faktor jenis pembiayaan. Analisis statistik yang digunakan adalah statistik deskriptif, distribusi varian dan analisis statistik uji beda. Hasil penelitian menunjukkan bahwa pasien CKD dengan tindakan hemodialisis yang masuk kriteria inklusi sebanyak 104 pasien dengan 1.861 episode rawat jalan dan 31 episode rawat inap. Cost of illness pasien CKD dengan tindakan hemodialisis sebesar Rp. 2.295.068.531,00. Komponen biaya terbesar pada pada pasien rawat jalan adalah biaya paket hemodialisis sebesar Rp. 1.374.690.150,00 (68%) dari total biaya medik langsung rawat jalan Rp. 2.025.849.359,00. Komponen biaya terbesar pada rawat inap yaitu biaya instalasi rawat inap sebesar Rp. 69.514.208,00 (32,2%) dari total biaya Rp. 215.771.440,00 untuk pasien rawat inap tanpa pembedahan. Komponen biaya terbesar untuk pasien rawat inap dengan pembedahan yaitu biaya farmasi sebesar Rp. 18.463.375,00 (34,5%) dari total biaya Rp. 53.447.732,00. Pada pasien rawat jalan terdapat perbedaan total biaya medik langsung pada faktor jenis kelamin, usia, jenis komorbid, dan frekuensi hemodialisis (p<0,05). Pada pasien rawat inap, tidak ada perbedaan antara faktor tersebut terhadap total biaya medik langsung.

The process of hemodialysis is an expensive treatment measures and will be a heavy burden for patients undergoing hemodialysis action repeatedly over a lifetime. The purpose of this study was to determine the total cost of the disease Chronic Kidney Disease (CKD) and to determine the differences in patient factors, disease factors, and factor in the type of financing to direct medical costs in patients with CKD on hemodialysis action outpatient and inpatient at Bethesda Hospital in Yogyakarta. This research uses descriptive analytic study design with cross-sectional study in a hospital perspective. The data were collected retrospectively by looking at the medical records of patients undergoing hemodialysis from January to June 2014. The variables in this study, namely, the dependent variable was the total direct medical cost of inpatient and outpatient hemodialysis action in Bethesda Hospitals Yogyakarta, while the independent variable was a factor of the patient, disease factors, and factor in the type of financing. The statistical analysis used descriptive statistics, statistical analysis of the distribution of variants and different test. The Results showed that patients with CKD on hemodialysis action that goes inclusion criteria, 104 patients premises 1,861 outpatient episodes and 31 episodes of hospitalization. Cost of illness CKD patients with hemodialysis action Rp. 2,295,068,531. The largest cost component in outpatient hemodialysis is the cost of the package of Rp. 1,374,690,150 (68%) of the total direct medical cost of outpatient Rp. 2,025,849,359. The biggest cost component is the cost of installation inpatient hospitalization Rp. 69,514,208 (32.2%) of the total cost of Rp. 215,771,440 for hospitalized patients without surgery. The largest costs for inpatients with surgery that pharmaceutical costs Rp. 18,463,375 (34.5%) of the total cost of Rp. 53,447,732. In outpatients there is a difference in total direct medical costs on the factors gender, age, comorbidities, and the frequency of hemodialysis (p <0.05). In hospitalized patients, there is no difference between these factors to total direct medical costs.
