PERBEDAAN EKSPRESI INTERLEUKIN-8 PADA MUKOSA ANTRUM DAN CORPUS LAMBUNG PENDERITA DISPEPSIA KRONIS DENGAN INFEKSI HELICOBACTER PYLORI DI RSUP DR. SARDJITO YOGYAKARTA

Latar belakang: IL-8 adalah neutrophil- activating chemokine poten yang diekspresikan sel epitel lambung dan berperan penting dalam proses peradangan dan pembentukan lesi di mukosa lambung. Gastritis kronik akibat infeksi H.pylori mengakibatkan gastritis predominan antrum (10-15%) dengan hyperchlorhydria menyebabkan ulkus peptikum, sedangkan jika predominan di corpus (2-5%) akan menginduksi gastritis atrofi menyebabkan hypochlorhydria. Tujuan Penelitian: Mengetahui perbedaan ekspresi IL-8 pada mukosa antrum dan corpus pasien dispepsia kronis dengan infeksi H.pylori. Metode: Penelitian ini menggunakan desain potong lintang. Dilakukan pemeriksaan ekspresi IL-8 pada preparat jaringan hasil biopsi endoskopi pasien dispepsia kronis dengan infeksi H.pylori pada bulan Januari 2009 sampai Maret 2014. Akurasi gradasi pada penelitian ini dinilai dengan melakukan uji kappa (nilai K ditentukan >= 0,75), sedangkan reliabilitas data dinilai dengan cronbach alpha (ditentukan >0,8). Data numerik-kategorik lebih dari 2 kelompok, tidak berpasangan di hitung dengan uji Mann-whitney U. Untuk distribusi data normal perhitungan dengan independent t-test, sedangkan distribusi tidak normal menggunakan uji Mann Whitney U dan uji Kruskal Wallis. Analisa data menggunakan program SPSS versi 17.0. Perbedaan dianggap bermakna jika didapatkan p < 0,05 dengan interval kepercayaan 95%. Hasil : Didapatkan 26 sampel (13 pasang sampel) preparat biopsi mukosa lambung terdiri 13 sampel mukosa antrum dan 13 sampel mukosa corpus lambung. Nilai median ekspresi IL-8 berdasarkan jumlah sel mononuklear yang mengekspresikan IL-8(%) pada antrum sebesar 83 (min18-maks96), sedangkan pada corpus sebesar 68 (min21-maks93). Dilakukan uji komparasi dengan uji Mann Whiytney U. Dari perhitungan statistik didapatkan nilai p=0,158 dengan p>0,05 Kesimpulan : Tidak didapatkan perbedaan yang signifikan secara statistik pada ekspresi IL-8 berdasarkan lokasi lesi mukosa lambung di antrum dan corpus pasien dengan dispepsia kronis dengan infeksi H.pylori di RSUP.Dr.Sardjito Yogyakarta.

Background: IL-8 is a potent neutrophil- activating chemokine expressed in gastric epithelial cells and plays an important role in the process of inflammation and the formation of lesions in the gastric mucosa. Chronic gastritis due to H.pylori infection resulted predominantly antral gastritis (10-15%) with hyperchlorhydria causes peptic ulcer, whereas if predominant in the corpus (2-5%) will induce atrophic gastritis cause hypochlorhydria. Objective: To identify differences the expression of IL-8 in the antrum and corpus mucosa ofchronic dyspepsia patients with H.pylori infection. Methods: This study used a cross-sectional design. Examined the expression of IL-8 in tissue endoscopic biopsies of chronic dyspepsia patients with H.pylori infection in January 2009 to March 2014. The accuracy of grading in this study assessed the test-kappa determined value (K>= 0.75), while reliability Data was assessed by Cronbach alpha (determined> 0.8). Numeric-categorical data more than 2 unpaired groups, calculated by Mann-Whitney U test, for normal data distribution calculations by independent t-test, while not normal distribution using the Mann Whitney U test and Kruskal Wallis test. Data were analyzed using SPSS version 17.0. Differences were considered significant if the obtained p <0.05 with 95% confidence intervals. Results: There were 26 samples (13 pairs of samples) gastric mucosal biopsy preparations comprising 13 antral mucosa samples and 13 samples of gastric corpus mucosa.The median value of the expression of IL-8 based on the number of mononuclear cells that express IL-8 (%) in the antrum was 83 (min18 - maks 96), while the corpus is 68 (min 21 - maks 93). From the results of a comparative test with Mann Whitney U test p value = 0.158 with p> 0.05. Conclusion: There were no statistically significant differences in the expression of IL-8 in the antrum and corpus gastric mucosa of patients chronic dyspepsia with H.pylori infection in RSUP.Dr.Sardjito Yogyakarta.
