RANCANGAN ULANG MAP (FOLDER) BERKAS REKAM MEDIS RAWAT JALAN DI UPT PUSKESMAS WONOSARI II

2015 | Tugas Akhir | D3 REKAM MEDIS SVLatar Belakang : Salah satu fungsi penyimpanan berkas rekam medis adalah untuk mempermudah dan mempercepat penyediaan berkas rekam medis yang disimpan dalam rak filing. Di UPT Puskesmas Wonosari II masih terdapat formulir rekam medis yang hilang, sobek dan tercecer. Kesulitan dalam pencarian berkas karena nomer rekam medis tidak terlihat jelas dikarenakan nomer rekam medis berada diatas kop map akibatnya nomor rekam medis tidak terlihat saat berkas berada dalam rak penyimpanan. Bahan map memakai kertas dupleks karton tipis dan manila tipis yang masih mudah rusak saat digunakan sebagai map berkas rekam medis. Pada map rekam medis juga belum terdapat tulisan tahun kunjangan yang bisa berguna untuk mengetahui tahun terakhir pasien berkunjung ke UPT Puskesmas Wonosari II dan belum terdapat kode warna. Tujuan : menghasilkan desain ulang map (folder) berkas rekam medis sesuai kebutuhan penggunanya, dan lebih mempermudah petugas dalam melakukan pekerjaan pada bagian penyediaan dan penyimpanan di UPT Puskesmas Wonosari II Proses Pembuatan Rancangan: Diawali dengan mengumpulkan data melalui observasi, wawancara, FGD I untuk mengetahui item-item apa saja yang dibutuhkan dan FGD II untuk mengetahui map berkas rekam medis yang seperti apa yang sesuai dengan keadaan di UPT Puskesmas Wonosari II dan dibantu dengan teori pendukung rancangan map berkas rekam medis. Membuat dua rancangan alternatif map untuk dipilih salah satu yang paling sesuai dengan keadaan dan kebutuhan. Hasil: Mengacu pada kebutuhan pengguna, fokus untuk melengkapi item pada map, mempermudah petugas dalam pengambilan dan penyimpanan berkas rekam medis, dan memperbaiki kualitas kertas yang lebih kuat dengan ukuran 22.5 x 33.5 cm. Miniatur dengan skala 1:1.5 Cetak 5000 dengan harga 2.000 rupiah/map sudah termasuk penjepit / fastener. Background: One of the functions of medical record folders storage was to provide the medical record folders easier and faster which were saved in filling racks. In UPT (integrated service unit) of the social health center Wonosari II, there were some lost, torn, or separated medical record forms. The difficulties to find those forms were because the medical record numbers could not be seen clearly since the medical record numbers were written above the folder head. That was why the medical record numbers could not be seen clearly when they were in the storage racks. The folders were made of sheer duplex carton and sheer manila which were easily broken to be used as medical record folders. On the medical record folders, there was not also written the visiting year which could be used to know the last year of the patient visited UPT (integrated service unit) of the social health center Wonosari II. Besides, there was not also color code. Objective: produce the re-plan design of medical record folder based on the users needs and make the staffs easier to do their jobs in the part of providing and saving the document in UPT (integrated service unit) of the social health center Wonosari II Designing process: it was started by collecting the data through observation, interview, FGD (Focus Group Discussion) I to find out the items which were needed and FGD (Focus Group Discussion) II to find out the kind of medical records which were suitable to the condition of UPT (integrated service unit) of the social health center Wonosari II and helped by the supporting theories about the re-plan of medical record folders. Then, it was made two alternate folder plans to be chosen one of the most suitable one to the condition and need. Results: based on the users needs, it focused on completing the items on the folders, facilitating the staffs to take and save the medical record folders, and improving the papers quality which was stronger and durable in the size of 22.5 x 33.5 cm.
