KORELASI KADAR SERUM MATRIX METALLOPROTEINASE 1 (MMP-1) DENGAN SKOR CHILD PUGH PADA PASIEN SIROSIS HATI

Sirosis hati merupakan gambaran akhir dari penyakit fibrosis hati oleh berbagai etiologi antara lain infeksi virus, parasit, alkoholisme, penyakit metabolik, autoimun dan drug induce / toxicity, dimana pasien mempunyai resiko kompilasi klinik berupa hipertensi portal, disfungsi hepatoseluler, dan keganasan hepatoseluler. Penelitian saat ini, untuk menilai progresifitas kemunduran fungsi hati pada sirosis hati ditujukan pada marker non invasive, diantaranya Matrix Metalloproteinase 1 (MMP-1). Tingkat Keparahan sirosis hati dalam klinis dilakukan dengan menggunakan skor Child-Pugh yang tidak terlepas dari subyektivitas dan inkonsistensi. Penelitian ini bertujuan menguji korelasi antara kadar serum MMP-1 dengan skor Child-Pugh. Metode penelitian ini menggunakan rancangan potong lintang terhadap pasien sirosis hati yang diambil secara berurutan di bangsal Penyakit Dalam dan pasien sirosis hati rawat jalan di poli Hepatogastro-Enterologi RS DR Sardjito Yogyakarta. Terhadap pasien dilakukan anamnesis, pemeriksaan fisik, dan pemeriksaan laboratorium. Uji distribusi data dengan Kolmogorov Smirnov diperoleh persebaran data normal. Kemudian digunakan uji beda untuk beda rerata masing masing kelompok Child-Pugh menggunakan uji Mann-Whitney. Untuk menilai korelasi kadar MMP-1 dengan tingkat keparahan sirosis berdasarkan skor Child-Pugh, digunakan korelasi Spearman. Perbedaan bermakna secara statistik ditetapkan dengan nilai p < 0,05 dan interval kepercayaan (IK) 95%. Dilakukan juga uji penampilan analitik untuk presisi dan akurasi. Besar sampel yang dipergunakan sebanyak 59 subjek. Penelitian ini menemukan adanya perbedaan yang signifikan (p<0,05) untuk rerata MMP-1, albumin, dan bilirubin total pada perbandingan kategori Child-Pugh A dan Child-Pugh B, Child-Pugh B dan Child-Pugh C, serta Child-Pugh A dan Child-Pugh C. Kemudian terdapat korelasi negatif antara MMP-1 dengan skor Child-Pugh (r= -0,808 p=0,000) pada penderita sirosis hati. Terdapat korelasi(korelasi negatif) yang bermakna antara MMP-1 dengan skor Child-Pugh.

Hepatic Cirrhosis is rerpresentation of the end histological from chronic hepatitis. Liver fibrosis disease due to several etiologies amongst others viral infection, parasites, alcoholism, metabolic disease, autoimmunity and drug induced / toxicity where patients are at risk for clinical complications consisting of portal hypertension, hepatocellular dysfunction and hepatocellular carcinoma. At present research to mark progression of the deteriorating function of the liver in liver cirrhosis aims for a non-invasive marker amongst MMP-1. The severity of liver cirrhosis is clinically measured using the Child-Pugh score but which be separated from subjectivity and inconsistency. This research aims to test the correlation between the level of MMP-1 serum and the Child-Pugh score. This research uses a cross cutting design of patients with liver cirrhosis which are taken in a specific order at the medical ward and out patients with liver cirrhosis at the Hepato-gastroenterology out patient clinic at Dr Sardjito Hospital in Yogyakarta, indonesia.An anamnesis is taken from the patients followed by a physical assessment and laboratory test. Test data distribution with the Kolmogorov Smirnov obtained normal data distribution. Then used different test for mean difference of each group Child-Pugh using Mann-Whitney. The sample size consists of 59 subjects to know the correlation between the level of MMP-1 serum and the Child-Pugh score using Spearman correlation to test the correlation. This research was find significantly (p<0,05) difference for the mean MMP-1, albumin, total bilirubin in comparison categories of the Child-Pugh A and Child-Pugh B, Child-Pugh B and Child-Pugh C, Child-Pugh A and Child-Pugh C. Then there is a negatif correlation between MMP-1 and the Child-Pugh score (r = -0,808 p=0,000) in patients with liver cirrhosis. This research was find a negatif correlation between MMP-1 and the Child-Pugh score in patients with liver cirrhosis.
