INTISARI Latar Belakang: Petugas assembling harus berusaha melakukan upaya untuk menjaga kelengkapan berkas rekam medis, dengan melakukan upaya tersebut, diharapkan bisa melengkapi ketidaklengkapan berkas rekam medis. Di RSJD Dr. RM Soedjarwadi Provinsi Jawa Tengah ingin mencapai tujuan tercapainya angka ketidaklengkapan Catatan Medis (KLPCM) 0%, tetapi berdasarkan hasil wawancara dengan Kepala Instalasi Rekam Medis dan studi dokumentasi berkas rekam medis pasien rawat inap diketahui jika tujuan tersebut masih belum tercapai karena masih ditemukan berkas rekam medis rawat inap yang tidak lengkap. Tujuan Penelitian: Mengetahui upaya yang dapat dilakukan untuk menjaga kelengkapan berkas rekam medis. Metode Penelitian: Jenis penelitian ini adalah penelitian deskriptif dengan pendekatan kualitatif serta rancangan penelitian cross sectional. Subjek dalam penelitian ini adalah petugas assembling, dua dokter, dua perawat, dan Kepala Instalasi Rekam Medis. Teknik pengambilan data yaitu observasi, wawancara, dan studi dokumentasi. Hasil Penelitian: Penyebab ketidaklengkapan berkas rekam medis yaitu a) Man: perawat lebih mengutamakan pelayanan atau penanganan terhadap pasien; perawat kurang patuh untuk mengisi lengkap berkas rekam medis; perawat masih kurang mengingatkan dokter bagian rekam medis yang harus diisi oleh dokter; dokter visit satu minggu sekali; dokter harus segera menangani pasien yang membutuhkan pertolongan. b) Method: prosedur pengisian rekam medis belum disosialisasikan kepada dokter ataupun perawat. c) Material: belum dimanfaatkannya secara maksimal adanya stiker identitas pasien dan jumlah stiker identitas pasien masih belum mencukupi. d) Machine: belum adanya fasilitas cap tanggal. e) Money: belum adanya sistem insentif apabila dalam pengisian rekam medis masih belum lengkap. Upaya melengkapi berkas rekam medis pasien rawat inap yaitu a) Man: petugas assembling; perawat; dokter. b) Method: petugas assembling memanggil atau mengembalikan berkas rekam medis yang bersangkutan ke bangsal rawat inap, petugas assembling tidak melakukan upaya melengkapi autentikasi perawat, autentikasi dokter pada selain Resume Pasien Pulang, dan penulisan waktu; perawat meminta dokter untuk melengkapi ketidaklengkapan berkas rekam medis pasien rawat inap; dokter membubuhkan tanda tangan pada Resume Pasien Pulang dan membuat Resume Pasien Pulang. c) Material: petugas assembling menempelkan stiker identitas pasien. d) Machine: Peralatan dalam bentuk mesin yang digunakan dalam upaya melengkapi berkas rekam medis pasien rawat inap yang dilakukan tidak ada, fasilitas yang digunakan yaitu bolpin. e) Money: belum ada sistem intensif dalam melakukan upaya melengkapi berkas rekam medis. Hambatan petugas assembling dalam melengkapi berkas rekam yaitu a) Man: terbatasnya tenaga perekam medis dan dokter visit satu kali dalam seminggu. b) Method: petugas harus mencari berkas kebangsal jika berkas rekam medis dibutuhkan untuk proses pelayanan karena saat berkas dikembalikan ke bangsal untuk dilengkapi, perawat tidak langsung mengembalikan berkas tersebut ke Instalasi Rekam Medis. c) Material: habisnya stiker identitas pasien. d) Machine: tidak ada hambatan dari segi machine. e) Money: tidak adanya sistem insentif jika berhasil menjaga kelengkapan berkas rekam medis pasien rawat inap. Abstract Background: assembling officer must strive to make efforts to maintain the completeness of medical record, with these efforts, is expected to complete the incomplete medical record file. In RSJD Dr. RM Soedjarwadi Provinsi Jawa Tengah wants to achieve the goal of achieving the numbers incompleteness medical record (KLPCM) 0%, but based on the result of interviews with the Chief Medical Record Installation and documentation of medical records of hospitalized patients is unknown if the goal has not been achieved as yet found medical record file hoaspitalization were incomplete. Research purpose: knowing the effort that can be done to maintain the completeness of the medical record. Research methode: this study was a descriptive approach and cross-sectional study design. Subject in this study were assembling officer, two doctors, two nurses, and the Chief Medical Record Installation. Data collection technique are observation, interviews, dan documentation. Research result: the cause of incomplete medical recoed file inpatients in RSJD Dr. RM Soedjarwadi Provinsi Jawa Tengah, namely a) Man: nurse prioritize care or treatment of the patient; nurses are less adherent to fill the complete medical records file; nurses still not remind part of medical records to the doctors to be filled; the doctors visite one a week; doctor should immediately treat patient. b) Method: the filled procedure of medical record has not been socialized. d) Material: the patient identification sticker has not been exploited and the sticker patient identity is still not sufficient. d) Machine: lact of facilities date stamp. e) money: lack of punishment in the form of incentives (money) when the fill medical record is still incomplete. Effort assembling officer complete medical record file inpatients namely a) Man: assembling officer; nurse; doctor. b) Methode: assembling officer call or return the nurse to complete medical records; the nurse asked the doctor for complete medical record; doctor sign the Resume and making Resume. d) Material: assembling officer patch the patient identification sticker. d) machine tool is the form of machine used in an effort to complete the medical record inpatient who do not exist, the facility used is bolpoin. e) Money: no intensive form of punishment and reword (money) in its efforts to complete the medical record file. Barriers assembling officer the complete medical record file inpatients is a) Man: lack of medical record personnel and the doctor visite once a week. b) Method: the nurse did not immadiately return the file to the installation of the medical record. d) material: endless sticker patient identity. d) Machine: there are no barrier in term of machine. e) Money: the absence of reword if successful completeness of medical record keeping inpatients.
