IMPLEMENTASI DISPENSING SAFETY PRACTICES DI INSTALASI RAWAT INTENSIF RSUP DR. KARIADI SEMARANG

Keselamatan pasien sangat penting dan sekarang mulai menjadi prioritas utama dalam pelayanan kesehatan terhadap masyarakat. Medication errors merupakan salah satu tipe error yang mendapat perhatian paling besar dalam usaha peningkatan kualitas dan keamanan pelayanan kesehatan. Medication errors merupakan tipe error yang paling sering terjadi dalam proses pelayanan kesehatan, khususnya pada tahap dispensing. Salah satu faktor penyebab dispensing erros adalah tingginya beban kerja. Penelitian ini dilakukan di Instalasi Rawat Intensif RSUP Dr.Kariadi selama 1 bulan melalui pendekatan studi cross sectional dengan melakukan observasi dan wawancara terhadap responden. Penelitian ini mengambil sampel sebanyak 802 resep dengan metode total sampling dari pasien yang dirawat inap di Instalasi Rawat Intensif RSUP Dr.Kariadi Semarang yang telah memenuhi kriteria inklusi dan eksklusi. Tujuan penelitian ini untuk mengklasifikasikan tentang jenis dispensing errors, mengidentifikasi jumlah dispensing errors dan mengetahui hubungan terjadinya dispensing errors dengan beban kerja petugas farmasi. Dari Hasil Penelitian diperoleh data 266 dispensing errors dari 802 total resep. Jenis dispensing errors yang terjadi adalah dengan urutan paling tinggi yaitu ketidaktepatan jumlah obat sebesar 7,23%, Ketidaktepatan jenis obat sebesar 3,74%, dan tidak dilakukan review terhadap resep sebesar 2,99 % adapun faktor yang berperan terhadap timbulnya insiden adalah lingkungan kerja sebesar 40,2%, faktor proses dispensing 32,1%, faktor petugas sebesar 27,7%. Faktor lingkungan kerja, khususnya beban kerja terbukti secara statistik (p= 0,000) memiliki hubungan yang signifikan dengan terjadinya dispensing errors . Patient safety is very important and that is now a top priority in health care to the community. Medication errors are one of the types of errors that got the most attention in efforts to improve the quality and safety of health care services Medication errors are the most frequent type of error that occurred in the process of health care services, particularly in the dispensing stage. One of the causes of dispensing errors is high workload. This research was conducted in intensive care Dr.Kariadi's Hospital Installation for a month through cross sectional approach to the study of observation and interviews with respondents. This study took a sample of 802 prescriptions of patients who are hospitalized in the intensive ward RSUP Dr.Kariadi Semarang and filtered by criteria inclusion and exclusion using total sampling method. The purpose of this study to classify on the type of dispensing errors, identify the number of dispensing errors and determine the correlation of the dispensing errors with pharmacy personnel workload. Research results obtained from the data of 266 dispensing error from 802 total prescriptions. Types highest of dispensing errors that occur is the inappropriateness amount of drug at 7,23%, inappropriateness type of drug 3,74%, and prescription is not being reviewed at 2,99% while the factors that contribute to the occurrence of the incident is the working environment by 40,2%, factor dispensing process 32,1%, and 27,7% a factor of officers.
