PERBANDINGAN HASIL LUARAN FUNGSI ABOVE ELBOW CASTING DAN BELOW ELBOW CASTING PADA FRAKTUR RADIUS DISTAL

Latar Belakang : Fraktur radius distal merupakan kasus yang paling sering ditemui pada fraktur ekstremitas atas atau kurang lebih seperenam dari semua kasus fraktur yang ditemui di unit gawat darurat. Pilihan terapi konservatif terhadap fraktur yang stabil dan pasien dengan low demand, baik secara above elbow casting maupun below elbow casting masih menjadi kontroversi sampai dengan saat ini. Metode : Penelitian Randomized clinical trial. Pasien memenuhi kriteria inklusi onset < 24 jam, usia > 30 tahun, trauma tunggal, fraktur tertutup, tanpa komorbiditas. Subjek dibagi menjadi 2 kelompok berdasarkan perlakuan above elbow casting dan below elbow casting. Casting dipertahankan selama 6 minggu lalu dinilai hasil luaran fungsi setelah 8 minggu setelah removal cast dengan Quick Dash Score Hasil : Pasien dengan fraktur radius distal n = 28 dengan usia rerata 51,8 yang terbagi dalam 2 kelompok perlakuan. Hasil analisa terhadap hasil luaran kedua kelompok (p = 0,308), Pada kelompok Above elbow casting p = 0,654 untuk usia, p= 0,272 untuk jenis kelamin. Sedangkan pada kelompok below elbow casting didapat p = 0,222 untuk usia dan p = 0,7 untuk jenis kelamin Kesimpulan : Tidak didapatkan perbedaan yang bermakna terhadap hasil luaran perlakuan above elbow casting maupun below elbow casting pada pasien dengan x fraktur radius distal. Faktor usia dan jenis kelamin juga tidak mempengaruhi secara bermakna terhadap hasil luaran. Kata Kunci : Fraktur radius distal, Above elbow casting, Below elbow casting, Quick Dash Score

Background: Distal radius fracture is the most common case in upper extremity fracture or approximately one-sixth of all fractures treated in emergency departments. Type of conservative treatment for stable and low demand patient between above elbow casting and below elbow casting are still becoming controversy Method: Randomized clinical trial, with inclusion criteria less than 24 hour of injury, age more than 30, single trauma, close fracture and without comorbidity. Subjects are divided into 2 groups, above elbow casting and below elbow casting. 6 weeks of casting period than evaluate after 8 weeks from cast removal with Quick Dash Score Result: Distal radius fracture patients n = 28 with mean age 51,8 devided into 2 groups of treatment. Analysis of the outcome of both groups (p = 0,308), Above elbow casting p = 0,654 for age factor, p = 0,272 for sex factor. In below elbow casting group p = 0,222 for age factor and p = ,7 for sex factor. Conclusion: No differences of outcome between above elbow casting and below elbow casting in treating distal radius fracture.
