PERBEDAAN EKSPRESI INTERLEUKIN-6 JARINGAN KANKER BERDASARKAN DERAJAT DIFERENSIASI ADENOKARSINOMA KOLOREKTAL

Latar belakang : Interleukin-6 (IL-6) adalah pleiotropic cytokine yang diproduksi oleh sel tumor yang berfungsi sebagai faktor pertumbuhan. Kanker kolorektal memproduksi IL-6 sehingga peningkatan IL-6 di mukosa colon merupakan suatu tanda pertumbuhan sel yang berlebihan. Derajat diferensiasi menjadi salah satu faktor prognostik dalam kanker kolorektal tetapi belum jelas apakah ekspresi yang berlebihan dari IL-6 jaringan kanker kolorektal berkaitan dengan perbedaan derajat diferensiasi tumor. Tujuan Penelitian : untuk mengetahui apakah terdapat perbedaan ekspresi IL-6 jaringan kanker antara diferensiasi baik, sedang dan buruk pada pasien adenokarsinoma kolorektal. Metode Penelitian : menggunakan desain cross sectional dan subjek berupa preparat biopsi reseksi adenokarsinoma kolorektal yang memenuhi kriteria inklusi dan eksklusi. Analisis statistik menggunakan Statistical Product and Service Solutions (SPSS) 20.0 for Windows. Hasil Penelitian : dari 42 sampel penelitian, rerata usia pasien 54,23± 13,29 tahun, subyek laki-laki 17 (40,5 %) orang. Jumlah pasien jaringan tumor dengan derajat diferensiasi baik 18 (42,9%), sedang 19 (45,2%) dan buruk 5 (11,9%). Nilai ekspresi IL-6 jaringan dengan diferensiasi baik sebesar 0,751, sedang 0,683 dan buruk 0,749. Tidak didapatkan perbedaan yang bermakna ekspresi interleukin-6 jaringan diantara kelompok diferensiasi baik vs sedang-buruk (p=0,477). Pada analisis subgrup juga tidak didapatkan perbedaan ekspresi IL-6 yang bermakna diantara kelompok diferensiasi baik, sedang dan buruk (p= 0,732). Kesimpulan : tidak terdapat perbedaan ekspresi IL-6 jaringan yang bermakna antara kelompok diferensiasi tumor yang baik, sedang dan buruk.

Background : Interleukin-6 (IL-6) had been shown to be a pleiotropic cytokine produced by tumor cell that act as growth factor. Colorectal cancer can produce IL-6, as a consequence there is a raising of IL-6 level in colon mucosa that shows excessive development of cell. Tumor differentiation grade had become one of prognostic factors in colorectal cancer but it is unclear whether the increase of IL-6 expression in colorectal cancer is related to tumor differentiation grade. Objectives :This reserach was held to find out the difference of tissue expression of IL-6 between well versus moderate and poor differentiation in colorectal adenocarcinoma. Methods : We performed a cross sectional study. The subject of this study was the resection preparation of colorectal tissue biopsies from all patients with adenocarcinoma colorectal with tissue samples that were sent to the department of Pathology Anatomy Dr. Sardjito between March 2010 and March 2014 who met the inclusion and exclusion criteria. Statistical analysis was done by Statistical Product and Service Solutions (SPSS) 20.0 for Windows. Results : Fourty two samples of this study had been analysed. This study consisted of 17 male subjects (40,5%) with mean age of 54,23 ±13,29 years old. Eighteen samples had well differentiation tumor grade (42,9%),19 samples had moderate differentiation (45,2%) and 5 samples had poor differentiation (11,9%). There was no significant difference interleukin-6 tissue expression between well versus moderate-poor differentiation (p=0,477). In subgroup analysis, there was also no significant difference of IL-6 tissue expression between well versus moderate versus poor differentiation group (p =0,732). Conclusion : There was no significant difference of tissue IL-6 expression between well versus moderate-poor differentiation group.
