Hubungan Ekspresi Endothelin-1 dengan Glomerulosklerosis pada Model 5/6 Nefrektomi Subtotal di Mencit

LATAR BELAKANG: Penyakit gagal ginjal kronis kini telah menjadi beban pada seluruh lapisan masyarakat. Pemeriksaan fungsi ginjal dengan mengukur kadar kreatinin serum masih didapatkan berbagai keterbatasan. Ekspresi endothelin-1 (ET-1) yang terlibat dalam patogenesis gagal ginjal kronis diduga menjadi penanda lain gagal ginjal kronis yang berhubungan dengan glomerulosklerosis yang terjadi TUJUAN : Untuk menganalisis korelasi antara ekspresi endothelin-1 dengan tingkat keparahan glomerulosklerosis pada mencit dengan gagal ginjal kronis yang diinduksi dengan perlakuan 5/6 nefrektomi subtotal. METODE: dua puluh satu ekor mencit (Mus musculus) Swiss background, usia 3-4 bulan, berat 30-40 gram yang akan diinduksi gagal ginjal kronis dengan perlakuan 5/6 nefrektomi subtotal (5/6 SN). Subyek mencit dibagi menjadi 3 kelompok perlakuan yaitu 5/6 SN 7 hari observasi (7N), 5/6 SN 28 hari observasi (28N), dan sham operation (SO) sebagai kontrol. Mencit diterminasi sesuai hari obsevasi. Ginjal dan darah mencit diambil untuk mengetahui skor keparahan glomerulosklerosis, ekspresi endothelin-1, dan kadar kreatinin serum. HASIL: Kadar kreatinin serum meningkat pada ketiga kelompok perlakuan (SO, 7N, dan 28N) dan nilainya berbeda secara signifikan(p < 0.01). Skor keparahan glomerulosklerosis juga meningkat secara signifikan antar kelompok perlakuan (p < 0.01). Terjadi peningkatan ekspresi ET-1 yang signifikan antara SO vs 7N (p < 0.05) dan antara SO vs 28N (p < 0.01). Sementara untuk peningkatan antara 7N terhadap 28N tidak signifikan. Hasil uji korelasi menunjukkan bahwa ada korelasi positif antara peningkatan ekspresi ET-1 dengan tingkat keparahan glomerulosklerosis (rho = 0.848; p < 0.01). KESIMPULAN : Terdapat korelasi positif antara peningkatan ekspresi endothelin-1 dengan peningkatan keparahan glomerulosklerosis pada mencit dengan gagal ginjal kronis yang diinduksi dengan perlakuan 5/6 nefrektomi subtotal.

BACKGROUND: Chronic kidney disease (CKD) is now become the burden of disease in every lining community. The examination of kidney function that measure creatinine serum level is still has a limitations. Endothelin-1 (ET-1) expression which is has a role in CKD pathogenesis is suspected to become another marker of chronic kidney disease that correlated with the development of glomerulosclerosis. OBJECTIVE : To analyze the correlation between ET-1 expression and glomerulosclerosis level in mice model suffering from CKD after 5/6 subtotal nefrectomy. METHOD: Twenty one mice 3-4 month age, 30-40 gram in weight, Swiss background were conducted induction CKD with 5/6 subtotal nefrectomy (5/6 SN). Mice were divided into 3 group of treatment: 5/6 SN, 7 days observation (7N), 5/6 SN, 28 days observation (28N), and sham operation (SO) as a control group. Mice terminated equal to the end of each observation day. Mice's kidney and blood are drawn to measure glomerulosclerosis score, ET-1 expression, and creatinine serum level. RESULT: creatinine serum level increase in 3 group (SO, 7N, dan 28N) and the increment increase significantly (p < 0.01). Glomerulosclerosis score level increase significantly between each group group (p < 0.01). There is increment in ET-1 expression significantly between SO vs 7N (p < 0.05) and between SO vs 28N (p < 0.01). While the increment between 7N to 28N is not significant. The result of correlation test showed that there is a positive correlation between the increment in ET-1 expression with the increment glomerulosclerosis score level (rho = 0.848; p < 0.01). CONCLUSION : There is a positive correlation between the increment in ET-1 expression with the increment glomerulosclerosis score level in mice with CKD induced by 5/6 subtotal nefrectomy.
