ANALISIS BIAYA TERAPI PADA PASIEN KANKER PAYUDARA DENGAN TERAPI HORMON DI RSUP DR. WAHIDIN SUDIROHUSODO MAKASSAR SULAWESI SELATAN

Penatalaksanaan kanker payudara salah satunya dilakukan dengan terapi hormon. Jenis terapi hormon yang diberikan kepada pasien mempengaruhi besarnya biaya yang diperlukan pasien. Tujuan penelitian ini adalah mengetahui komponen biaya terapi dan besar biaya masing-masing unit untuk terapi rawat jalan dan rawat inap dan mengetahui pengaruh karakteristik pasien, faktor terapi dan jenis terapi hormon terhadap biaya terapi kanker payudara di RSUP Dr. Wahidin Sudirohusodo. Metode penelitian secara observasional analitik menurut perspektif rumah sakit, dengan rancangan penelitian cross sectional. Subyek penelitian adalah pasien kanker payudara rawat jalan dan rawat inap dalam periode Januari - Juni 2014 di RSUP Dr. Wahidin Sudirohusodo yang memenuhi kriteria inklusi. Pengambilan data dilakukan secara retrospektif yang diambil dari penelusuran dokumen catatan medik dan data biaya pengobatan pasien kanker payudara berdasarkan perspektif rumah sakit. Data yang digunakan yaitu nomor rekam medik, usia, jenis kelamin, diagnosis primer kanker payudara, stadium kanker payudara, komorbid, jenis terapi, lama terapi dan data tentang obat. Analisis data dilakukan dengan menghitung prevalensi pasien kanker payudara yang diberikan terapi hormon, komponen penyusun biaya dan besar biaya untuk masing-masing komponen, baik untuk terapi rawat jalan maupun rawat inap. Hasil penelitian menunjukkan bahwa pasien kanker payudara yang masuk dalam kriteria inklusi sebanyak 105 pasien yang menjalani rawat inap dan rawat jalan. Pada biaya rawat inap pembedahan memiliki rentang biaya yaitu Rp. 5.436.756 – Rp. 11.071.481 dengan komponen biaya terbesar pada rawat inap pembedahan yaitu bahan medis habis pakai (BHP) yang termasuk dalam biaya tindakan medik sebesar Rp. 3.161.748 (28,6%) dari biaya total Rp. 11.071.481. Biaya rawat inap kemoterapi memiliki rentang biaya yaitu Rp. 2.546.166 – Rp. 6.823.821. Komponen biaya terbesar pada rawat inap kemoterapi yaitu biaya obat sebesar Rp. 5.701.138 (83,5%) dari biaya total Rp. 6.823.821. Biaya rawat jalan memiliki rentang biaya yaitu Rp. 1.538.750 – Rp. 4.202.935. Komponen biaya terbesar pada rawat jalan yaitu biaya obat sebesar Rp. 2.495.000 (63,57%) dari biaya total Rp. 4.202.935. Karakteristik pasien yang mempengaruhi besarnya biaya penyakit kanker payudara yaitu stadium (p = 0,001), tindakan medik (p = 0,001), dan siklus terapi (p = 0,027). Selain itu jenis terapi hormon juga mempengaruhi besarnya biaya penyakit kanker payudara (p = 0,047).

Management of breast cancer is one done with hormone therapy. Type of hormone therapy given to patients affect the cost of the required patient. The purpose of this study was to determine the major cost component of therapy and the cost of each unit for outpatient therapy and inpatient therapy and determine the influence of patient characteristics, factor of therapy, and type of hormone therapy on breast cancer treatment costs at RSUP. Dr. Wahidin Sudirohusodo. The method is an analitic observational study, with a cross-sectional study design. Subjects were patients with breast cancer outpatient and inpatient in the period January-June 2014 at RSUP. Dr. Wahidin Sudirohusodo who met the inclusion criteria. Data were collected retrospectively extracted from medical records and document tracking data is the cost of treatment of breast cancer patients from hospital perspektive. The data used is the medical record number, age, sex, primary diagnosis of breast cancer, breast cancer stage, comorbidities, type of therapy, duration of therapy, and data on the drug. Data analysis was performed by calculating the prevalence of breast cancer patients are given hormone therapy, major components of costs and expenses for each component, both for therapy outpatient or inpatient. The results showed that breast cancer patient which included in inclusion criteria is 105 patient who underwent inpatient and outpatient. The cost of inpatient for surgery has a range of cost is Rp. 5.436.756 - Rp. 11.071.481 with the largest cost component in inpatient surgery is medical substance after use (BHP) which included in the cost of medical action is Rp. 3.161.748 (28.6%) from total cost Rp. 11.071.481 . The cost of inpatient chemotherapy has a cost range of Rp. 2.546.166 - Rp. 6.823.821 with the largest cost component in inpatient surgery is the cost of drugs is Rp. 5.701.138 (83.5%) from total cost Rp. Rp. 6.823.821. The cost of outpatient has a cost ranges of Rp. 1.538.750 - Rp. 4.202.935 with the largest cost component in outpatient is the cost of drugs is Rp. 2.495.000 (63.57%) from total cost Rp. 4.202.935. Patient characteristics that influence the cost of breast cancer is stage (p = 0.001), medical action (p = 0.001), and cycles of therapy (p = 0.027). In addition, the type of hormone therapy also affects the cost of breast cancer (p = 0.047).
