PERBEDAAN KADAR SMALL DENSE LOW DENSITY LIPOPROTEIN ANTARA INFARK MIOKARD AKUT NON ELEVASI SEGMEN ST DAN INFARK MIOKARD AKUT DENGAN ELEVASI SEGMEN ST

Di Amerika, terdapat 13,2 juta kasus sindroma koroner akut (SKA), dimana 33% kasus merupakan kasus infark miokard akut dengan elevasi segmen ST (IMA-EST) dan 45% kasus merupakan infark miokard akut non elevasi segmen st (IMA-nEST). Small dense low density lipoprotein (sd-LDL) merupakan salah satu faktor risiko SKA. Namun, belum ada penelitian yang melihat perbedaan kadar sd-LDL pada pasien IMA-nEST dan IMA-EST. Tujuan Penelitian : Untuk mengetahui apakah terdapat perbedaan kadar sd-LDL yang bermakna antara pasien IMA-nEST dan pasien IMA-EST Metode Penelitian : Penelitian ini merupakan penelitian observasional bersifat potong lintang dengan menggunakan data mengenai lipid atherogenik plasma dan SKA yang sudah ada sebelumnya. Penghitungan kadar sd-LDL dilakukan menggunakan rumus. Diagnosis IMA-EST dan IMA-nEST ditentukan berdasar pedoman ACCF/AHA. Hasil Penelitian : Subyek yang diteliti berjumlah 186 pasien IMA yang terdiri dari 52 pasien IMA-nEST dan 134 pasien IMA-EST. Analisis statistika menunjukkan rerata kadar sd-LDL pada pasien IMA-nEST adalah sebesar 33,57 ± 13,37 mg/dL dan sebesar 36,92 ± 12,90 mg/dL pada pasien IMAEST dimana tidak terdapat perbedaan kadar sd-LDL yang bermakna (p>0,05) antara pasien IMA-nEST dan pasien IMA-EST (p=0,118). Kesimpulan : Tidak terdapat perbedaan kadar sd-LDL yang bermakna antara pasien IMA-nEST dan pasien IMA-EST.
Background: Coronary heart disease (CHD) is the leading cause of death in many countries, including Indonesia. In America, there are 13.2 million cases of acute coronary syndrome (ACS), where 33% of cases are cases of ST segment elevation myocardial infarction(STEMI) and 45% of cases are non ST segment elevation myocardial infarction (NSTEMI). Small dense low-density lipoprotein (sd-LDL) is one of the risk factors for ACS. However, there is no study about the level difference in sd-LDL between patients with STEMI and NSTEMI. Research Objective: To determine whether there is a significant difference of the sd-LDL levels between NSTEMI and STEMI. Research Methods: This study is a cross-sectional observational study using former data from atherogenic plasma lipid and ACS's study. Calculation of sd-LDL levels was performed using a formula provided. Diagnosis of NSTEMI and STEMI is based on ACCCF/AHA's guideline. Research Result : There are 186 AMI as subjects studied consisting of 52 patients with NSTEMI and 134 patients with STEMI. Statistical analysis showed the average level of sd- LDL in patients with NSTEMI is 33.57 ± 13.37 mg/dL and 36.92 ± 12.90 mg / dL in patients with STEMI. There are no significant differences in the levels of sd-LDL (p> 0.05) between NSTEMI and STEMI.(p = 0.118). Conclusion: There is no significant difference in sd-LDL levels between NSTEMI and STEMI.
