PENGARUH KONSENTRASI DAN LAMA WAKTU APLIKASI SODIUM ASKORBAT TERHADAP KEBOCORAN MIKRO TUMPATAN RESIN KOMPOSIT KAVITAS KELAS I PASCA BLEACHING INTRAKORONAL DENGAN HIDROGEN PEROKSIDA 35%

Perawatan bleaching intrakoronal dengan menggunakan hidrogen peroksida 35% dapat meningkatkan kebocoran mikro pada tumpatan resin komposit. Salah satu cara untuk mengurangi kebocoran mikro adalah dengan pengaplikasian sodium askorbat. Tujuan dari penelitian ini adalah untuk mengetahui pengaruh konsentrasi (10% dan 25%) dan lama aplikasi (1 hari dan 3 hari) sodium askorbat terhadap kebocoran mikro tumpatan resin komposit pasca bleaching intrakoronal dengan hidrogen peroksida 35%. Dua puluh lima spesimen gigi premolar satu mandibula dipotong 2 mm ke arah apikal dari CEJ, dipreparasi berbentuk kerucut terbalik pada oklusal diameter 3 mm, pada apikal diameter 2 mm. Barrier cervical diaplikasikan setebal 2 mm diapikal dari CEJ dengan ionomer kaca modifikasi resin (IKMR) kapsul. Spesimen gigi dibagi secara acak menjadi 5 kelompok,masing-masing kelompok berjumlah 5 gigi. Kelompok 1 (kontrol) adalah bleaching intrakoronal dengan hidrogen peroksida (H2O2) 35% 3 hari selama 3 kali, kelompok 2 – 5 dilakukan bleaching seperti kelompok kontrol kemudian diaplikasi sodium askorbat (SA) pada kelompok 2: SA 10% selama 1 hari, kelompok 3 : SA 10% selama 3 hari, kelompok 4 : SA 25% selama 1 hari, kelompok 5 : SA 25% selama 3 hari. Semua kelompok dimasukkan ke dalam inkubator 370C. Gigi ditumpat dengan resin komposit teknik incremental, direndam dalam larutan metilen biru 2% selama 24 jam, kemudian dilihat kebocoran mikro dengan menggunakan stereomikroskop. Hasil uji Anava dua jalur menunjukkan terdapat perbedaan signifikan nilai kebocoran mikro pada lama aplikasi sodium askorbat (p<0,05), tidak terdapat perbedaan signifikan nilai kebocoran mikro pada konsentrasi sodium askorbat (p>0,05), dan terdapat interaksi antara lama aplikasi dan konsentrasi sodium askorbat. Hasil LSD menunjukkan terdapat perbedaan yang bermakna antara lama aplikasi sodium askorbat 1 hari, 3 hari, dan kontrol. Pada penelitian ini disimpulkan bahwa kebocoran mikro tumpatan resin komposit pada gigi pasca bleaching dengan hidrogen peroksida 35% yang diaplikasi sodium askorbat lebih kecil dibandingkan tanpa aplikasi sodium askorbat. Aplikasi sodium askorbat 10% dan sodium asorbat 25% pada gigi pasca bleaching dengan hidrogen peroksida 35% tidak menurunkan kebocoran mikro tumpatan resin komposit. Kebocoran mikro pada gigi pasca bleaching dengan hidrogen peroksida 35% yang diaplikasi sodium askorbat 3 hari lebih kecil dibandingkan 1 hari. Intracoronal bleaching using 35% hydrogen peroxide can increase microleakage in composite resin filling. To reduce microleakage can be done by the application of sodium ascorbate. The purpose of this study was to determine the effect of concentration sodium ascorbate 10% and 25% also application period sodium ascorbate 1 day and 3 days on microleakage of composite resin filling after it was bleached with 35% hydrogen peroxide. Twenty five specimens of mandibular fisrt premolar teeth was cut 2 mm apical direction of the CEJ, prepared with inverted cone bur, diameter of occlusal cavity was 3 mm, and diameter of apical cavity was 2 mm. Cervical barrier was applied 2 mm thickness in the apical direction of CEJ with resin modified glass ionomer (RMGI). Tooth specimens were randomly divided into 5 groups, each group contained 5 teeth. Group 1 (control) was applied with hydrogen peroxide (H2O2) 35% 3 days for 3 times, groups of 2-5 were bleached with hydrogen peroxide 35% then various concentration and application period of sodium ascorbate (SA) were applied. Group 2 was applied 10% of SA for 1 day. Group 3 was applied 10% of SA for 3 days. Group 4 was applied 25% of SA for 1 day. Group 5 was applied 25% of SA for 3 days. All groups were put in 370C incubator. After that the teeth were filled with composite resin using incremental technique, soaked in 2% methylene blue solution for 24 hours, then the microleakage was evaluated using stereomicrosope. Two-way Anova showed significant difference in microleakage after various application period of sodium ascorbate (p<0.05), there was no significant difference in microleakage values in concentration of sodium ascorbate (p> 0.05), and there was interaction between the time application and the concentration of sodium ascorbate. LSD test showed that there was significant difference between application period of sodium ascorbate: 1 day, 3 days, and control. From this research it could be concluded that the microleakage of composite resin filling after it was bleached with 35% hydrogen peroxide followed by the application of sodium ascorbate less than those without the application of sodium ascorbate. The usage of 10% and 25% of sodium ascorbate after bleaching with 35% hydrogen peroxide did not reduce the microleakage of composite resin. Microleakage of composite resin filling after bleaching with 35% hydrogen peroxide that followed by application of sodium ascorbate for 3 days was below the one that was applied for 1 day.
